Objectives: Suicidal thoughts are among the major concerns of medical students. These thoughts may serve as a precursor to suicidal attempts. Suicidal ideation is associated with emotional regulation abilities and early childhood experiences. The present study aimed to evaluate a model explaining the relationship between adverse childhood experiences and suicidal thoughts, with the mediating role of emotion regulation difficulties and metacognitive beliefs among medical interns
Methods: This was a fundamental research study using structural equation modeling. The statistical population included all medical interns at Mashhad University of Medical Sciences in 2023. A total of 300 medical students were selected through convenience sampling. Participants completed the Adverse Childhood Experiences Questionnaire, the Difficulties in Emotion Regulation Scale, the Metacognitions Questionnaire, and the Beck Scale for Suicidal Ideation. Data analysis was conducted using SPSS-25 and AMOS-24 software.
Results: The results showed that adverse childhood experiences had a significant positive effect on suicidal thoughts (β = 0.471, p < 0.01), emotion regulation difficulties (β = 0.762, p < 0.01), and metacognitive beliefs (β = 0.807, p < 0.01). Emotion regulation difficulties (β = 0.271, p < 0.01) and metacognitive beliefs (β = 0.267, p < 0.01) also had significant positive effects on suicidal thoughts. Moreover, the indirect effects of adverse childhood experiences on suicidal thoughts through emotion regulation difficulties (β = 0.359, p < 0.01) and metacognitive beliefs (β = 0.441, p < 0.01) were also significant.
Conclusion: Adverse childhood experiences have a direct effect on suicidal thoughts, and this relationship is mediated by difficulties in emotion regulation and metacognitive beliefs.
