Objectives: Suicidal thoughts are among the major concerns of medical students. These thoughts may serve as a precursor to suicidal attempts. Suicidal ideation is associated with emotional regulation abilities and early childhood experiences. The present study aimed to evaluate a model explaining the relationship between adverse childhood experiences and suicidal thoughts, with the mediating role of emotion regulation difficulties and metacognitive beliefs among medical interns
Methods: This was a fundamental research study using structural equation modeling. The statistical population included all medical interns at Mashhad University of Medical Sciences in 2023. A total of 300 medical students were selected through convenience sampling. Participants completed the Adverse Childhood Experiences Questionnaire, the Difficulties in Emotion Regulation Scale, the Metacognitions Questionnaire, and the Beck Scale for Suicidal Ideation. Data analysis was conducted using SPSS-25 and AMOS-24 software.
Results: The results showed that adverse childhood experiences had a significant positive effect on suicidal thoughts (β = 0.471, p < 0.01), emotion regulation difficulties (β = 0.762, p < 0.01), and metacognitive beliefs (β = 0.807, p < 0.01). Emotion regulation difficulties (β = 0.271, p < 0.01) and metacognitive beliefs (β = 0.267, p < 0.01) also had significant positive effects on suicidal thoughts. Moreover, the indirect effects of adverse childhood experiences on suicidal thoughts through emotion regulation difficulties (β = 0.359, p < 0.01) and metacognitive beliefs (β = 0.441, p < 0.01) were also significant.
Conclusion: Adverse childhood experiences have a direct effect on suicidal thoughts, and this relationship is mediated by difficulties in emotion regulation and metacognitive beliefs.

Introduction and Objectives: The increasing prevalence of disordered eating behaviors and their transformation into eating disorders, along with their comorbidity with other psychological disorders, makes it essential to identify the underlying and perpetuating factors contributing to these behaviors. The aim of this study was to investigate the mediating role of self-compassion and distress tolerance in the relationship between insomnia severity and disordered eating behaviors in college students.
Methods: This study employed a correlational structural equation modeling design. A total of 385 students were selected through convenience sampling and completed online questionnaires, including the Dutch Eating Behavior Questionnaire (DEBQ), Insomnia Severity Index (ISI), Self-Compassion Scale-Short Form (SCS-SF), and Distress Tolerance Scale (DTS). Data analysis was conducted using SPSS-22 for preliminary analysis and AMOS-24 for the structural equation.
Results: The findings demonstrated that the direct path from insomnia severity to disordered eating behaviors was statistically significant (t =2.22, β = 0.20). Bootstrap analyses further revealed a significant indirect effect of insomnia severity on disordered eating behaviors through self-compassion (p < 0.05, b= 0.51). In contrast, the indirect effect through distress tolerance did not reach statistical significance (p > 0.05, b = -0.18).
Conclusion: The findings highlight the significant role of insomnia severity and self-compassion in predicting disordered eating behaviors. These findings highlight the potential value of interventions targeting sleep quality and self-compassion to prevent and reduce disordered eating behaviors.
These results underscore the importance of addressing insomnia severity and promoting self-compassion in therapeutic and preventive interventions for individuals exhibiting disordered eating behaviors.

Introduction:
Organ donation is a life-saving medical intervention performed to preserve life or improve a patient’s physical condition. In this procedure, a healthy organ is removed from the body of a living person, a person with brain death or heart death and transplanted to the body of a sick person who has a defective organ(1-6). In cases where transplantation follows death, organ donation requires the consent of the deceased individual’s family(5, 7-9). Evidence suggests that organ donation may be associated with psychological consequences for donor families, recipients, or both(10-13). Following donation, donor families may wish to obtain information about the fate of the donated organ(14-20), while recipients may seek contact with donor families to express gratitude(14, 17, 21). However, the process of communication between donor families and recipients remains unclear and poorly defined in most parts of the world(14, 15, 17, 21, 22). Moreover, donation teams, hold divergent views regarding whether and how such contact should occur(10-12). This study aimed to systematically review the psychological consequences of meeting between organ donor families and recipients.

Methods: Following PRISMA guidelines, we searched English (Scopus, PubMed, Web of Science) and Persian (Magiran, SID) databases for studies published between 1980 and December 2022 to examine the psychological consequences of contact between donor families and organ recipients.
Two independent reviewers screened the identified studies based on predefined inclusion and exclusion criteria. Studies lacking either a “meeting” component or “psychological consequences” were excluded. Inter-rater agreement was assessed using Cohen’s kappa coefficient, yielding κ = 0.82 at the title/abstract screening stage and κ = 0.87 at the full-text review stage, indicating excellent agreement. Discrepancies were resolved through discussion, and when necessary, a third reviewer adjudicated.
Following screening, seven studies were included in the content analysis, and findings related to psychological consequences of Meeting between donor families and recipients were extracted. Study quality was assessed using the CASP tool for qualitative studies, and the Newcastle–Ottawa Scale for quantitative studies. Overall, the included studies were of acceptable quality ( Table1).
Results:
According to the duration between the initial awareness of potential contact and the actual meeting, the findings were classified into two primary domains(Table 1).

The pre- contact phase(from awareness to meeting):

Negative consequences experienced by donor families
	Negative consequences experienced by recipients

	The Post- meeting phase:

	Consequences for donor families

Positive consequences (e.g., reduced ambiguity, facilitated grieving, enhanced meaning-making, and more positive attitudes toward donation)
Negative consequences (e.g., reactivation of grief, emotional dependency)

	Consequences for recipients

Positive consequences (e.g., gratitude, psychological relief)
Negative consequences (e.g., guilt, anxiety about graft survival, symbolic identification with the deceased)

Conclusion:
This systematic review conceptualizes meeting between donor families and organ recipients as a complex, multifaceted phenomenon encompassing both beneficial and adverse psychological consequences. On the positive side, such contact may facilitate meaning-making, emotional closure, and expressions of gratitude. Conversely, it may give rise to emotional dependency, feelings of ownership, and the reactivation of grief, highlighting its potential psychological risks.

The findings are predominantly derived from Western societies, individualistic contexts, thereby limiting cross-cultural generalizability. In contrast, Eastern and religious societies, such as Iran, are shaped by collectivist norms and belief systems that may significantly influence how such meeting is perceived and experienced. In these settings, implicit or explicit social expectations to initiate or maintain communication may emerge, potentially leading to psychological consequences that differ from those observed in Western contexts. These considerations underscore the necessity of culturally sensitive research and the development of context-specific, supplementary guidelines.

A key limitation in the literature is the predominance of cross-sectional and short-term studies. The psychological impact of meeting is inherently dynamic, as grief processing, adaptation to organ donation, and interpersonal relationships evolve over time. However, longitudinal data remain scarce, indicating a critical gap in understanding long-term trajectories and delayed effects. Therefore, future studies should prioritize longitudinal designs to better capture these long-term effects.

Methodologically, both qualitative and quantitative approaches have been employed. While qualitative studies provide in-depth insights, and quantitative studies enhance generalizability, mixed-methods designs with larger samples are needed. Overall, contact should not be universally recommended, and evidence-based, culturally adapted guidelines are essential.
Accordingly, the following considerations should be incorporated into guideline development:

Organ donation regulations vary across countries. For example, in some jurisdictions, cross-border organ donation is permitted, which may influence the process of contact.
Contact guidelines should be grounded in principles of protection, confidentiality, autonomy, and informed choice for both donor families and recipients.
In light of cultural differences and their impact on psychological outcomes, each country should develop culturally adapted, context-specific protocols alongside adherence to general international principles.
Prior to initiating meeting, a comprehensive assessment of both parties (donor families and recipients) should be conducted, including evaluation of grief status, mental health, and expectations regarding contact. In cases of significant psychological vulnerability, meeting should be postponed or conducted with appropriate professional support.
Specialized training for donation and transplant teams, along with the involvement of rehabilitation counseling professionals, may improve mediation processes and reduce negative outcomes.
Individuals seeking meeting, should receive education regarding potential positive and negative outcomes, relational boundaries, privacy considerations, realistic expectation management, and the importance of preserving the recipient’s independent identity.

Ultimately, the decision to facilitate contact should not be universally mandated but rather guided by evidence-based, culturally adapted protocols that prioritize the psychological well-being of all stakeholders.

Objectives: Given the rapid growth of digital data and computational tools in mental health, there is an increasing need to systematically review the current evidence on the use of advanced technologies. This review aimed to examine the evolution of suicide risk identification and prevention through the application of artificial intelligence (AI) and machine learning (ML) algorithms.
Methods: A systematic review was conducted by retrieving relevant articles from Google Scholar, PubMed, ProQuest, EMBASE, PsycINFO, Web of Science, CINAHL, and Scopus, covering the period from January 2014 to September 2025. In total, 751 articles published in English were identified according to the predefined inclusion criteria and were reported in accordance with the PRISMA guidelines. Following a comprehensive quality appraisal using a standardized checklist, 25 studies with the highest methodological quality were selected for final inclusion and detailed analysis.
Results: The studies utilized diverse data sources, including text messages, social media platforms, electronic health records, and psychiatric clinical notes, with sample sizes ranging from a few hundred to several million records. AI and ML approaches applied included logistic regression, random forest, decision trees, XGBoost, and deep neural networks, with prediction accuracies ranging from 65% to over 95%. Key risk factors identified across studies included psychiatric disorders, depression, method of attempt, marital status, low education, unemployment, and social isolation. Random forest and deep neural networks consistently demonstrated the highest predictive performance.
Conclusion: AI and ML algorithms can detect complex patterns of suicide risk in both clinical and social media data, enabling early prediction and timely intervention. Random forest and deep neural network models provided the highest accuracy and practical tools for designing effective preventive strategies.

Objective:
The aim of this study was to examine the relationship between event centrality and post-traumatic growth, with the mediating roles of resilience and rumination, among women diagnosed with breast cancer who were referred to Omid and Imam Reza hospitals in Mashhad.
Method:
This cross-sectional study employed a structural equation modeling approach. The statistical population consisted of women with breast cancer who visited Omid and Imam Reza hospitals in Mashhad during the year 2023–2024. A total of 110 participants were selected using purposive sampling. Data were collected using the Event Centrality Questionnaire by Berntsen and Rubin (2006), the Post-Traumatic Growth Inventory by Tedeschi (2006), the Connor-Davidson Resilience Scale (2003), and the Rumination Questionnaire by Cann et al. (2011). Data analysis was conducted using SPSS 25 and Smart PLS 4, employing descriptive statistics and structural equation modeling.
Results:
Resilience served as an effective mediator, facilitating the relationship between event centrality and post-traumatic growth. Findings related to rumination indicated that deliberate rumination could indirectly influence post-traumatic growth by affecting resilience, whereas intrusive rumination did not show a significant effect in this regard.
Conclusion:
Overall, resilience plays a key role in facilitating post-traumatic growth among women with breast cancer. Deliberate rumination may indirectly influence post-traumatic growth by enhancing resilience.

Objectives: Cannabis use disorder is characterized by intense craving and impairments in executive functions, including inhibitory control and cognitive flexibility. These two factors may interact in a reciprocal cycle that sustains substance use and increases the risk of relapse. The present study aimed to compare the efficacy of repetitive transcranial magnetic stimulation (rTMS), computerized cognitive rehabilitation (CR), and their combined intervention on executive functions, craving, and neurophysiological indices in individuals with cannabis use disorder.
Materials and Methods: This study was conducted as a randomized controlled trial with a 2×2 factorial design. Forty individuals with cannabis use disorder, with a mean age of 32.4±5.6 years, were randomly assigned to four groups: combined rTMS+CR, rTMS only, CR only, and control. The interventions were delivered over four weeks in 20 sessions. Executive functions, craving, and quantitative electroencephalographic indices were assessed at baseline, at the end of Week 4, and at the Week 8 follow-up. Data were analyzed using mixed-design analysis of variance and two-way factorial analysis of variance.
Results: The combined intervention group showed the greatest improvement in inhibitory control, as measured by the Stroop test (F(6,72)=6.45, p<0.001, ηp²=0.30), and in cognitive flexibility, as measured by the Trail Making Test-Part B (F(6,72)=7.12, p<0.001, ηp²=0.37). Craving decreased by approximately 20% in the combined group, compared with 12.9% in the CR-only group, 4.7% in the rTMS-only group, and 3.5% in the control group. QEEG findings showed significant increases in theta, beta, and gamma power in prefrontal and frontal regions, together with a sustained increase in parietal alpha power. Delta-band functional connectivity between frontal and parietal regions was also strengthened in the combined group, and some of these changes persisted at the four-week follow-up. However, in the factorial analysis, the rTMS×CR interaction was statistically significant only for parietal alpha power (F(1,36)=4.63, p=0.038, ηp²=0.11) and did not reach statistical significance for craving, Stroop performance, or TMT-B performance.
Conclusion: The combined rTMS and cognitive rehabilitation intervention was associated with greater improvements in neurophysiological indices, executive functions, and craving than either intervention alone. Nevertheless, statistical evidence of synergy was limited to parietal alpha power, and the observed superiority in behavioral outcomes cannot be interpreted as definitive evidence of a synergistic effect. Given the small sample size and the absence of objective measures of actual cannabis consumption, these findings should be confirmed in larger studies with longer follow-up periods and biological verification of substance use.
