			<CONTENT>هدف: این پژوهش با هدف بررسی اثربخشی گروه&#8204;درمانی فراشناختی&#173; بر مؤلفه&#173;های فراشناختی و تاب&#173;آوری زنان مبتلا به مولتیپل اسکلروزیس انجام گرفت. روش: این مطالعه نیمه&#173;آزمایشی، با استفاده از طرح پیش&#173;آزمون- پس&#173;آزمون با گروه کنترل به همراه دو مرحله پیگیری انجام شد. از بین تمام بیماران مبتلا به مولتیپل اسکلروزیس مراجعه&#173;کننده به انجمن MS خراسان رضوی در سال 1393، 30 نفر به روش نمونه&#173;گیری در دسترس انتخاب و به طور تصادفی در دو گروه آزمایش و کنترل قرار داده شدند. گروه آزمایش طی 8 هفته تحت درمان فراشناختی قرار گرفتند، درحالی&#8204;که گروه کنترل روند عادی جلسه&#173;های انجمن MS را می&#173;گذراندند. پرسشنامه&#173;های فراشناخت 30 سؤالی (MCQ-30) و تاب&#8204;آوری کانر- دیویدسون (CD-RISC) در پیش&#8204;آزمون، پس&#8204;&#8204;آزمون و پیگیری&#173;ها مورد استفاده قرار گرفت. داده&#173;ها با استفاده از تحلیل واریانس با اندازه&#8204;گیری مکرر تجزیه&#8204;وتحلیل شد. یافته&#173;ها: نتایج نشان دادند که گروه&#8204;درمانی فراشناختی منجر به بهبود معنی&#8204;دار تمام مؤلفه&#173;های فراشناختی (001/0p&#60;) و افزایش معنی&#173;دار تاب&#173;آوری (001/0&#62;p) در مقایسه با گروه کنترل می&#173;شود و این نتیجه پس از دو مرحله پیگیری یک&#173;ماهه و سه&#173;ماهه کماکان حفظ گردید. نتیجه&#173;گیری: گروه&#8204;درمانی فراشناختی منجر به بهبود فراشناخت&#173;ها و تاب&#173;آوری زنان مبتلا به مولتیپل اسکلروزیس می&#173;شود.</CONTENT>

			<CONTENT>Objectives: This study aimed to investigate the effectiveness of group metacognitive therapy on metacognitive components and resiliency of women with multiple sclerosis. Method: This semi-experimental study was carried out using pretest- posttest design with a control group and a two-step follow-up. From among all the patients with multiple sclerosis referred to the MS Society in Khorasan-e-Razavi in 2015, 30 individuals were selected via convenience sampling method and were randomly assigned to experimental and control groups. The experimental group received group metacognitive therapy for eight weeks while the control group attended the usual sessions of the MS Society. Metacognition Questionnaire-30 (MCQ-30) and Connor and Davidson Resiliency Scale (CD-RISC) were used at pretest, posttest and follow-ups. The data were analyzed using analysis of variance with repeated measures. Results: The results indicated that group metacognitive therapy leads to a significant improvement in all metacognitive components (p&#60;0.001) and a significant increase in resiliency (p&#60;0.001). This result was maintained after one- and three-month follow-ups. Conclusion: Group metacognitive therapy can improve metacognitions and resilience of women with multiple sclerosis.</CONTENT>

			<CONTENT>هدف: هدف پژوهش حاضر تبین نقش واسطه&#173;ای&#173; آمیختگی شناختی در تبیین ارتباط بین بی&#173;نظمی&#173; هیجانی با اضطراب و افسردگی بود. روش: در پژوهش حاضر 543 دانشجوی دانشگاه تهران به پرسشنامه آمیختگی شناختی (CFQ)، مقیاس مشکل در تنظیم هیجان (DERS)، پرسشنامه اضطراب بک (BAI) و پرسشنامه افسردگی بک-نسخه دوم (BDI-II) پاسخ دادند. تجزیه &#8204;وتحلیل داده&#173;ها با روش همبستگی پیرسون و رگرسیون چندگانه سلسله مراتبی انجام شد. یافته&#8204;ها: تمام متغیرهای پژوهش به&#8204;طور معنی&#173;داری با هم رابطه داشتند (001/0p&#60;). تحلیل رگرسیون چندگانه سلسله مراتبی نشان داد که آمیختگی شناختی نقش واسطه&#173;ی کامل در ارتباط بین متغیر بی&#173;نظمی هیجانی با شدت اضطراب (001/0&#62; p، 84/0= &#223;) و شدت افسردگی (001/0&#62; p، 79/0= &#223;) ایفا می&#173;کند و نتایج آزمون سوبل هم این نقش واسطه&#173;ای آمیختگی شناختی را برای اضطراب (001/0&#62; p، 35/6= z) و افسردگی (001/0&#62; p، 11/5= z) تأیید کرد. نتیجه&#173;گیری: آمیختگی شناختی یک متغیر واسطه&#173;ای در رابطه بین بی&#173;نظمی هیجانی با اضطراب و افسردگی محسوب می&#173;شود و با کنترل این متغیر، ارتباط بین بی&#173;نظمی هیجانی با اضطراب (05/0&#60; p، 09/0= &#223;) و افسردگی (05/0&#60; p، 07/0= &#223;) معنادار نیست.</CONTENT>

			<CONTENT>Objectives: The aim of the present study was to investigate the mediating role of cognitive fusion in the relationship of emotional dysregulation with anxiety and depression. Method: 543 students from Tehran University completed Cognitive Fusion Questionnaire (CFQ), Difficulties in Emotion Regulation Scale (DERS), Beck Anxiety Inventory (BAI) and Beck Depression Inventory-II (BDI-II). The data were analyzed using Pearson correlation and hierarchical multiple regression analysis. Results: All variables were correlated with each other significantly (p&#60;0.001). The hierarchical multiple regression analysis showed that cognitive fusion has a complete mediating role in the relationship between emotional dysregulation and intensity of anxiety (&#223;= 0.84, p&#60;0.001) and depression (&#223;= 0.79, p&#60;0.001). Also Sobel&#8217;s test results confirmed the mediating role of cognitive fusion for anxiety (z=6.35, p&#60;0.0001) and depression (z=5.11, p&#60;0.001) Conclusion: Cognitive fusion is a mediating variable in the relationship between emotional dysregulation with anxiety and depression. When this variable is controlled, the relationships between emotional dysregulation with anxiety (&#223;=0.09, p&#62;0.05) and depression (&#223;=0.07, p&#62;0.05) are no longer significant.</CONTENT>

			<CONTENT>هدف: مطالعه حاضر با هدف بررسی تأثیر آموزش بخشایش در احترام به همسر و رضایت زناشویی در زنان دچار مشکلات زناشویی انجام گرفت. روش: پژوهش آزمایشی و از نوع پیش آزمون- پس آزمون و پیگیری با گروه کنترل بود. جامعه آماری پژوهش شامل کلیه زنان دچار مشکلات زناشویی مراجعه کننده به مرکز مشاوره شهرستان بابل و نمونه پژوهش شامل 30 نفر از این زنان بود که با روش نمونه&#173;گیری در دسترس انتخاب شدند و پس از همتاسازی به صورت تصادفی در دو گروه آزمایش و کنترل قرار گرفتند. آزمودنی&#173;های دو گروه مقیاس&#173;های احترام به همسر و رضایت زناشویی را در مراحل پیش آزمون، پس آزمون و پیگیری تکمیل کردند. آزمودنی&#173;های گروه آزمایش شش جلسه&#173; آموزش بخشایش دریافت کردند و گروه گواه در فهرست انتظار بودند و مداخله&#173;ای دریافت نکردند. داده&#173;ها با استفاده از آزمون آماری تحلیل کوواریانس چندمتغیری (مانکوا) مورد تجزیه و تحلیل قرار گرفت. یافته&#173;ها: نتایج نشان داد که این مداخله به طور معنی&#173;داری باعث افزایش میزان احترام به همسر و رضایت زناشویی در گروه آزمایش در مقایسه با گروه کنترل، در مرحله پس آزمون و پیگیری چهار ماهه، شده است (001/0P&#60;). نتیجه&#173;گیری: آموزش بخشایش به عنوان یک رویکرد شناختی- رفتاری با ایجاد تغییر در شناخت و رفتار فرد، نقش مهمی در بهبود روابط زوجین دارد.</CONTENT>

			<CONTENT>Objectives: The aim of the present research was to determine the effect of forgiveness training on respect to spouse and marital satisfaction in women with marital problems. Method: This is an experimental study with pre-and post-test and follow-up with a control group. The population consisted of all the married women referred to Babol city counselling center. The sample included 30 married women selected using convenience sampling method and after matching were randomly assigned to either experimental or control group. Both groups completed the respect to spouse and marital satisfaction questionnaires at pre-and post-test, and follow-up. The experimental group received forgiveness training for six 90-minute sessions and control at group were placed on a waiting list and received no intervention. Data were analyzed using multivariate analysis of covariance (MANCOVA). Results: The results showed a significant effect of forgiveness training on promotion of respect to spouse and marital satisfaction in experimental group compared to controls at post- test and at four-month follow-up (p&#60;0.001). Conclusion: Forgiveness training as a cognitive behavioral approach through changing the individual&#8217;s cognition and behavior has an important role in improving couples&#8217; relations.</CONTENT>

			<CONTENT>Objectives: The high rate of comorbidity between bipolar and obsessive-compulsive disorders can be due to common transdiagnostic processes. Therefore, the aim of the present study was to determine the role of cognitive emotion regulation strategies (CERS), anxiety and impulsivity, in developing and maintaining bipolar and obsessive-compulsive disorders. Method: Twenty five patients with bipolar disorder (11 females and 14 males), 25 with comorbid bipolar disorder and obsessive-compulsive disorder (12 females and 13 males) and 25 healthy participants (13 females and 12 males) were selected using convenience sampling method. To collect the data, participants were administered Cognitive Emotion Regulation Questionnaire (CERQ-short), Impulsivity Questionnaire, and Beck Anxiety Inventory (BAI). Data were analyzed using multivariate analysis of variance. Results: Results showed that compared to healthy participants, patients with comorbid bipolar and obsessive-compulsive disorders use more self-blaming and other-blaming maladaptive cognitive emotion regulation strategies (p&#60;0.01). Compared with normal subjects, both patient groups utilized more rumination and catastrophizing strategies, the scores of the patients with comorbid disorder were higher than bipolar patients without comorbid on these strategies disorder (p&#60;0.01). Both patient groups used more adaptive positive refocusing strategies, reappraisal, and less putting into perspective compared to healthy controls(p&#60;0.01). Also, both patient groups scored higher than healthy controls in anxiety, attentional/cognitive impulsivity, motor impulsivity and non-planning impulsiveness; while patients with comorbid disorder scored higher than patients with only bipolar disorder (p&#60;0.01). Conclusion: Anxiety, impulsivity and some of the cognitive emotion regulation strategies (self-blaming, rumination, catastrophizing) are transdiagnostic processes and play critical roles in developing and maintaining bipolar disorder with and without obsessive-compulsive disorder.</CONTENT>

			<CONTENT>هدف: هدف مطالعه حاضر تعیین کیفیت خواب و ارتباط آن با سلامت عمومی در دستیاران سال اول و دوم رشته&#8204;های مختلف تخصصی پزشکی شاغل در مراکز آموزشی-درمانی تحت پوشش دانشگاه علوم پزشکی ایران بود. روش: این مطالعه یک بررسی مقطعی و توصیفی-تحلیلی بود. از بین 1246 دستیار سال اول و دوم رشته&#8204;های مختلف پزشکی دانشگاه علوم پزشکی ایران، تعداد 330 نفر (1/56 درصد مرد؛9/43 درصد زن؛ میانگین سن 9/31 سال) به&#8204;طور تصادفی انتخاب و بررسی شدند. ابزار جمع&#8204;آوری اطلاعات شامل پرسشنامه محقق&#8204;ساخت برای کسب اطلاعات جمعیت&#8204;شناختی و عادات دستیاران، پرسشنامه کیفیت خواب پیتسبورگ (PSQI) و پرسشنامه سلامت عمومی 28 سؤالی (GHQ-28) بود. برای تحلیل داده&#8204;ها از ضریب همبستگی پیرسون و تحلیل رگرسیون لجستیک چند اسمی استفاده شد. یافته&#8204;ها: متوسط نمره پرسشنامه کیفیت خواب پیتسبورگ (&#177;انحراف معیار) در این افراد 5/0&#177;9/6 بود (میانه 5).200 نفر (6/60 درصد) نمرات بالاتر از 5 داشتند که نشان&#8204;دهنده کیفیت نامطلوب خواب بود. نمرات کیفیت خواب همبستگی نسبتاً قدرتمندی با نمرات زیرمقیاس&#8204;های اضطراب و بی&#8204;خوابی و نیز نمره کل GHQ-28 (در تمام موارد: 6/0&#62;r؛ 001/0&#62;p) داشت. از بین متغیرهای زمینه&#8204;ای، تعداد 8 تا 12 کشیک در ماه (360/3=adjusted OR؛ فاصله اطمینان 95 درصد: 750/1 تا 451/6) و بیشتر از 12 کشیک در ماه (020/2=adjusted OR؛ فاصله اطمینان 95 درصد: 007/1 تا 053/4) نسبت به کمتر از 8 کشیک در ماه و سلامت عمومی نامناسب در GHQ-28 (141/9=adjusted OR؛ فاصله اطمینان 95 درصد: 006/4 تا 862/20) نسبت به سلامت عمومی مناسب با افزایش احتمال کیفیت نامطلوب خواب مرتبط بودند. نتیجه&#8204;گیری: بیش از نیمی از دستیاران سال اول و دوم دچار اختلالاتی در کیفیت خواب بودند و کیفیت خواب رابطه معنی&#8204;دار و معکوسی با وضعیت سلامت عمومی و تعداد کشیک&#8204;های ماهانه داشت.</CONTENT>

Of 1246 first- and second-year medical residents 330 (56.1% male, 43.9% female with mean age of 31.9) were selected randomly to participate in the study. The data collection instruments included a demographic questionnaire and habits of residents, the Pittsburgh Sleep Quality Index (PSQI), and the General Health Questionnaire-28 (GHQ-28). The data were analyzed using Pearson&#39;s correlation coefficient and multi-nominal logistic regression analysis. Results: Mean score of PSQI (&#177;SD) of studied individuals was 6.9&#177;0.5, (median 5). In total 200 persons (60.6%) had scores higher than 5 indicating undesirable sleep quality. Sleep quality scores had relatively strong correlation with subscales&#8217; scores of anxiety and insomnia; and also with GHQ-28 total score (r&#60;0.6; p&#60;0.001). Of all background variables, 8 to 12 shifts per month (adjusted OR=3.360; 95% CI: 1.750 to 6.451) and more than 12 shifts per month (adjusted OR=2.020; 95% CI: 1.007 to 4.053) compared to fewer than 8 shifts per month, and poor general health in GHQ-28 (adjusted OR=9.141; 95% CI: 4.006 to 20.862) compared to adequate general health were related to higher probability of inadequate sleep quality. Conclusion: More than half of the first-and second-year residents were experiencing poor sleep quality and sleep quality had significant and inverse correlation with general health status and number of shifts per month.</CONTENT>

			<CONTENT>هدف: این پژوهش به بررسی ویژگی&#173;های روان&#173;سنجی مقیاس احساس تنهایی کودک در دانش&#8204;آموزان می&#173;پردازد. روش&#173;: جامعه آماری شامل دانش&#173;آموزان ابتدایی دختر شهر کرمان (93-92) است که از میان آن&#173;ها 320 نفر با روش نمونه&#173;گیری تصادفی چند&#173;مرحله&#173;ای خوشه&#173;ای انتخاب شدند. برای گردآوری داده&#173;ها مقیاس احساس تنهایی به دانش&#173;آموزان و پرسشنامه سیاهه رفتاری کودک به مادرانشان جهت پاسخ&#160; گویی داده شد. برای تحلیل داده&#173;ها از آلفای کرونباخ، همبستگی پیرسون، تحلیل رگرسیون، همبستگی دو نیمه کردن وبازآزمایی استفاده شد. یافته&#173;ها: علاوه بر روایی صوری، مقیاس احساس تنهایی از همسانی درونی بالایی (83/0)برخوردار است و همبستگی&#173;های ماده-کل نشان داد، تمام ماده&#173;ها با نمره کل، همبستگی قابل قبولی دارند (57/0-20/0). اعتبار مقیاس از طریق دو نیمه کردن آزمون73/0 و روش بازآزمایی برابر با 76/0بود. روایی همزمان این مقیاس به&#173;واسطه همبستگی متوسط (25/0-18/0) آن با اکثر زیر مقیاس&#173;های درونی سازی شده سیاهه رفتاری کودک تأیید شد و احساس تنهایی پیش&#173;بینی&#173;کننده زیر&#173;مقیاس&#173;های سیاهه رفتاری کودک بود. نتیجه&#173;گیری: مقیاس احساس تنهایی کودک در جامعه دانش&#173;آموزان دختر دوره دبستان شهر کرمان دارای روایی و پایایی مناسبی است.</CONTENT>

			<CONTENT>Objectives: This study examined the psychometric properties of the Children&#8217;s Loneliness Scale among elementary school girls. Method: The statistical population consisted of elementary school girls in the city of Kerman (2013-2014), from which a sample of 320 students were selected using multistage cluster random sampling. For gathering data, Loneliness Scale to children and Child Behavioral Checklist were given to their mothers. Data were analyzed using Cronbach&#8217;s alpha, Pearson correlation, regression analysis, Split-half correlation, and test-retest. Results: In addition to face validity, the Children&#8217;s Loneliness Scale has high internal consistency (0.83) and item-total correlations showed that all items have acceptable correlation with the total score (0.20-0.57). Split-half and test-retest correlation coefficients were 0.73 and 0.76, respectively. Concurrent validity of the scale was confirmed by moderate correlations (0.18-0.25) with most of the internalizing subscales of Child Behavioral Checklist; and Loneliness was predicting factor for subscales of Child Behavioral Checklist. Conclusion: The Children&#8217;s Loneliness Scale in the community of the elementary school students in the city of Kerman has a good reliability and validity.</CONTENT>

			<CONTENT>هدف: صفات سه&#173;گانه تاریک شخصیت، فرمول&#173;بندی جدیدی از شخصیت ناسازگارانه است که از سطوح غیر بالینی خودشیفتگی، ماکیاولیسم و جامعه&#173;ستیزی تشکیل شده است. هدف پژوهش حاضر بررسی خصوصیات روان&#173;سنجی نسخه کوتاه پرسشنامه دوجین کثیف در جامعه ایرانی بود. روش: در این مطالعه مقطعی 300 دانشجو در سال تحصیلی 92-93 به روش نمونه&#173;گیری تصادفی طبقه&#173;ای انتخاب و با مقیاس صفات تاریک شخصیت، پرسشنامه&#173; شخصیت خودشیفته (NPI-16)، مقیاس جامعه&#173;ستیزی پرسشنامه شخصیتی چندوجهی مینه سوتا MMPI و پرسشنامه ماکیاولیسم- 5 مورد ارزیابی قرار گرفتند. یافته&#173;ها: تحلیل عاملی به روش تحلیل مؤلفه&#8204;های اصلی نشانگر وجود سه عامل خودشیفتگی، ماکیاولیسم و جامعه&#173;ستیزی بود. ضرایب باز آزمائی برای کل مقیاس و مؤلفه&#8204;ها در دامنه&#173;ای بین 66/0 تا 80/0 قرار داشت. ضریب همسانی درونی خرده مقیاس&#8204;ها بین 68/0 تا 77/0 به دست آمد. همچنین ضرایب همبستگی نمره کل صفات تاریک شخصیت با خودشیفتگی، 57/0؛ با جامعه&#8204;ستیزی، 42/0 و با ماکیاولیسم، 55/0 بود. درنهایت بین مردان و زنان در مؤلفه&#8204;های صفات تاریک شخصیت تفاوت وجود داشت. نتیجه&#8204;گیری: نسخه کوتاه پرسشنامه دوجین کثیف برای کاربرد در جامعه ایرانی از ویژگی&#173;های روان&#173;سنجی مناسب برخوردار است.</CONTENT>

			<CONTENT>Objectives: Dark triad is a new formulation of maladaptive personality that is composed of Machiavellianism, subclinical narcissism, and subclinical psychopathy. The aim of the current research was to study the psychometric properties of the short form of Dirty Dozen Scale among Iranian population. Method: In this cross sectional study, 300 university students in 2014-15 academic year were selected using stratified sampling method and were assessed by Dark Triad Scale, Narcissistic Personality Inventory-16 (NPI-16), Psychopathy scale of the Minnesota Multiphasic Personality Inventory (MMPI), and Machiavellianism Questionnaire (MACH-IV). Results: Factor analysis by principle component analysis extracted three components: narcissism, machiavellianism and psychopathy. Test-retest correlation coefficients for the total scale and its subscales ranged from 0.66 to 0.80. Internal consistency of the subscales ranged from 0.68 to 0.77. Also, correlation coefficients between total score of Dark Triad Scale and Narcissistic personality, psychopathy, and Machiavellianism were 0.57, 0.42, and 0.55, respectively. Finally, there was a significant difference between males, and females&#8217; scores on Dark Triad components. Conclusion: Short form of Dirty Dozen Scale has suitable psychometric properties for Iranian population.</CONTENT>

بخش اول تاریخچه انستیتو روانپزشکی تهران به روایت دکتر ایرج سیاسی در شماره قبلی مجله روانپزشکی و روانشناسی بالینی ایران چاپ شد (1).

تاریخچه انستیتو روانپزشکی تهران

شانزدهم مرداد ماه 1357، هم&#8204;زمان با فضای انقلابی در ایران، دکتر احمد محیط به همراه خانواده، با آرزوی خدمت به ایران و شوق سازندگی به کشور مراجعت کرد. روز پس از مراجعت به ایران، دکتر محیط با &#160;دکتر ایرج سیاسی که رئیس انجمن توانبخشی بود، ملاقات داشت و او، دکتر محیط را به&#8204;عنوان مسئول دستیاران روانپزشکی بیمارستان رازی معرفی کرد.

&#160;انجمن توانبخشی مشتمل بر یک واحد پژوهش و یک واحد آموزش بود. مسئول واحد پژوهش، دکتر بهمن فزونی&#160; بود و مطالعه گسترده&#8204;ای را روی همه&#8204;گیرشناسی بیماری&#8204;های روانی در ایران شروع کرده بود که دسترسی به مدارک آن بعداً ممکن نشد. واحد آموزش در دو بخش دستیاری روانپزشکی و آموزش روان&#8204;پرستاری مشغول به فعالیت بود و بخش روانشناسی بالینی در حال راه&#8204;اندازی و بخش آموزش کاردرمانی در حال شکل&#8204;گیری بود. مبنای کار واحد آموزش، تشکیل تیمی &#8204;متشکل از روانپزشک، روان&#8204;پرستار، روانشناس بالینی و کاردرمانگر1 بود و استراتژی و رسالت انجمن توانبخشی، ایجاد مراکز جامع خدمات بهداشت روان2 برگرفته از طرح معروف کندی3 در زمینه تحول بهداشت روان در آمریکا بود. سه مرکز در شهر تهران دایر شده بود. در مرکز، شرق و جنوب تهران؛ اما راه اندازی چنین مراکزی حتی در تهران نیز با موفقیت بالایی همراه نبود. دکتر محمود بهفر و دکتر داریوش ملک نیازی از روانپزشکانی بودند که در این مراکز مشغول به فعالیت بودند. مسئول واحد آموزش در سال&#8204;های قبل از پیروزی انقلاب، دکتر کیومرث فرد&#160; دانش&#8204;آموخته روانپزشکی از آمریکا بود. پس از انقلاب، شادروان دکتر احمد نظام از طرف شادروان دکتر کاظم سامی، وزیر بهداشت وقت، به سمت ریاست انجمن توانبخشی منصوب شد. در اردیبهشت 1358، مرحوم دکتر کاظم سامی &#8204;به&#8204;دنبال تقاضای کتبی برخی از دستیاران، از دکتر احمد محیط دعوت کرد تا به&#8204;عنوان مسئول واحد آموزش انجمن توانبخشی مشغول به کار شود و او شرط پذیرش دعوت آنها را رضایت دکتر کیومرث فرد اعلام کرد. دکتر ایرج سیاسی تا سال 1359 در ایران بود و در طی دو سال بعد از انقلاب، دکتر احمد محیط تمامی &#8204;تلاششان را برای حفظ شأن و حرمت ایشان در انجمن به عمل آورد.

انجمن توانبخشی در همان اوایل سال 1358 منحل شد ولی با درایت و پشتکار دکتر احمد محیط، واحدهای آموزشی و پژوهشی آن، در دل ساختار جدیدی به نام &#171;انستیتو روانپزشکی تهران&#187; قرار گرفت تا ادامه&#8204;دهنده راه و رسالتی باشد که انجمن توانبخشی با ریاست دکتر ایرج سیاسی آغازگر آن بود.

بدین ترتیب انستیتو روانپزشکی تهران در سال 1358 تحت نظارت معاونت آموزشی وزارتخانه و به ریاست دکتر احمد محیط افتتاح شد، ولی در نشان&#8204;واره (لوگو) انستیتو&#160; به پاس تلاش&#8204;های دکتر ایرج سیاسی و همکارانشان در انجمن توانبخشی، سال 1356 (سال آغاز به کار واحد&#8204;های آموزشی و پژوهشی انجمن) نقش بست. آرم یا نشان&#8204;واره انستیتو توسط دکتر محیط طراحی شده است. نمادهای به&#8204;کار رفته در این نشان&#8204;واره، حال و هوای فضای انقلابی آن سال&#8204;ها را تداعی می&#8204;کند؛ زنجیرها از یک سو به زنجیر استبداد و از سوی دیگر به زنجیر بسته شده به پای بیماران مبتلا به اختلالات روانپزشکی اشاره دارد و لاله&#8204;ها نیز در دو معنا به کار گرفته شده اند؛ یکی نمادی از خون شهیدان وطن و دیگری لاله&#8204;ای که باید به جای زنجیر، به پای بیماران روانپزشکی بسته شود و همه این&#8204;ها با مصرعی از شاعر پرآوازه و اندیشمند ایرانی، مولوی مزین می&#8204;شود: &#171;تن ز جان و جان ز تن مستور نیست&#187;.

بدین&#8204;ترتیب انستیتو روانپزشکی تهران از سال 1358 به&#8204;طور رسمی &#8204;آغاز به کار کرد و فعالیت&#8204;های انجمن به شکلی نو پیگیری شد. در ابتدای فعالیت انستیتو، دکتر کاظم سامی &#8204;از دکتر احمد محیط برای رفع بحرانی در بیمارستان روانپزشکی سلامی &#8204;شیراز دعوت کرد؛ به سرپرستی دکتر محیط، تعدادی از دستیاران روانپزشکی (با نمایندگی دکتر محمدعلی دربندی) و روان&#8204;پرستاران انستیتو ظرف مدت 24 ساعت از تهران به شیراز منتقل شده و بحران مذکور را مدیریت کردند.

اواخر سال 1358، انستیتو از ساختمانی در خیابان انقلاب، به خیابان آیت&#8204;اله طالقانی، ابتدای کوچه جهان نقل مکان کرد. انستیتو که در ابتدا تحت نظارت معاونت آموزشی وزارتخانه شروع به کار کرده بود در جریان تحولات، به علت مکان جغرافیایی اش، تحت نظارت اداره بهداری منطقه شرق تهران قرار گرفت.

در راستای سیاست عدم تمرکز بیماران در بیمارستان روانپزشکی رازی، انستیتو در پی گسترش مراکز بیمارستانی&#8204;اش برآمد. در سال 1360، باشگاه وزارت کشاورزی بر فراز تپه&#8204;های گیشا (کوی نصر)، در مکانی که امروزه برج میلاد تهران بر آن واقع شده، با حمایت دکتر عباس شیبانی (وزیر کشاورزی وقت) و تلاش دکتر احمد محیط، به انستیتو اختصاص داده و نام بیمارستان شهید اسماعیلی بر آن گذاشته شد. سپس باشگاه ساواک، واقع در کیلومتر 7 جاده مخصوص کرج، در زمان نخست وزیری شهید محمد علی رجایی و با وساطت مرحوم شگرف نخعی، به انستیتو ملحق و نام بیمارستان شهید نواب صفوی بر آن گذارده شد. شهید رجایی در برگه مجوز تأسیس این بیمارستان این جمله را مرقوم کردند: &#171;برای بیماران مستضعف روانی مبارک باشد&#187;. نام فعلی این مرکز، &#171;مرکز آموزشی درمانی روانپزشکی ایران&#187; است.

از دیگر فعالیت&#8204;های دکتر احمد محیط در زمان تصدی ریاست انستیتو روانپزشکی تهران، برگزاری جلساتی با دستیاران روانپزشکی شب&#8204;های یکشنبه در مطب بوده است. در این جلسات مباحث بین رشته&#8204;ای از فلسفه و ادبیات مورد مطالعه قرار می&#8204;گرفت.

برنامه سطح&#8204;بندی خدمات بهداشتی در قالب &#171;خدمات بهداشتی اولیه[1]&#187; به صورت آزمایشی (پایلوت) در دو منطقه ایران، قبل از پیروزی انقلاب (سال 1356) آغاز شده بود و دکتر محمدعلی برزگر، دکتر حسین ملک&#8204;افضلی، مرحوم دکتر کامل شادپور و مرحوم دکتر سیروس پیله&#8204;رودی از فعالان انجام طرح پایلوت بودند. در جوامعی مثل شوروی سابق و چین، از بین مردم هر منطقه، افرادی بومی &#8204;بودند که به سایر مردم، خدمات بهداشتی ارائه می&#8204;کردند؛ ایده بهورز تا حدودی از این جا شکل می&#8204;گیرد اما باید خاطر نشان کرد که پایه&#8204;گذار مفهوم (concept) بهورز در ایران، دکتر فریدون رهنما است.

با پیروزی انقلاب، برنامه مذکور در سطح پایلوت متوقف ماند تا اینکه در زمان اولین دوره وزارت دکتر سید علیرضا مرندی، به&#8204;صورت یک طرح ملی و کشوری به اجرا درآمد. موفقیت این طرح که یکی از برجسته&#8204;ترین رویدادها در تاریخ پزشکی ایران در سال&#8204;های بعد از انقلاب است مرهون تلاش&#8204;های دکتر حسین ملک افضلی، مرحوم دکتر کامل شادپور و مرحوم دکتر سیروس پیله&#8204;رودی در همراهی با دکتر سید علیرضا مرندی بود.

پس از اجرای این طرح عظیم، اولین شخصی که از سازمان جهانی بهداشت، پس از انقلاب از ایران بازدید به&#8204;عمل آورد دکتر ویگ[2] بودند، ایشان مسئول وقت بهداشت روان در &#160;[3](EMRO)بودند و این بازدید در سال 1364 انجام شد. ]شاید بتوان گفت سفر دکتر ویگ به ایران بهانه&#8204;ای شد تا دغدغه&#8204;ها و ایده&#8204;های دکتر محیط جامه عمل بپوشند. در گفتگوی مفصلی که &#171;رسانه داروپخش&#187; (شماره 55/ سال پنجم/ مهرماه 1364) پیش از بازدید دکتر ویگ از ایران با دکتر احمد محیط انجام داد، نگرش و دیدگاه ایشان به خوبی نمایان است. سی سال پیش که جمعیت ایران حدود چهل میلیون نفر و تعداد روانپزشکان حدود 200 نفر بود، نظرات و پیشنهادات ایشان در این مصاحبه از یک سو شامل &#171;تشکیل تیم روانپزشکی مشتمل بر روانپزشک، روان&#8204;پرستار، مددکار اجتماعی، روانشناس و کاردرمانگر&#187;، &#171;نیاز مبرم به تشکیل شبکه خدمات روانپزشکی در تلفیق با شبکه بهداشتی- درمانی کشور&#187;، &#171;سطح&#8204;بندی خدمات روانپزشکی از سطح روستا به سطح مرکز استان&#187;، &#171;ایجاد مراکز روزانه&#187; و &#171;حمایت از بیماران روانپزشکی در قوانین کار&#187; در جهت بیماریابی و از سوی دیگر &#171;تداوم درمان و حفظ سلامت روانی بیماران روانپزشکی خارج از بیمارستان و در جامعه&#187; بود[. دکتر ویگ طرح آزمایشی ادغام بهداشت روان در مراقبت&#8204;های بهداشتی اولیه را در هند انجام داده بود. در طول مدت اقامت یک هفته&#8204;ای او در ایران، به پیشنهاد دکتر احمد محیط، پیش&#8204;نویس برنامه ملی بهداشت روان کشور با هدایت انستیتو روانپزشکی و مشارکت تنی چند از روانپزشکان ایرانی نوشته شد. تهیه&#8204;کنندگان پیش&#8204;نویس برنامه ملی بهداشت روان علاوه بر دکتر ویگ از سازمان جهانی بهداشت عبارت بودند از دکتر احمد محیط (سازمان&#8204;دهنده و دبیر برنامه)، دکتر &#8204;هاراطون داویدیان (رئیس جلسات)، دکتر محمد صنعتی، دکتر ابوالقاسم حسینی، دکتر نصرت&#8204;اله پورافکاری، دکتر سید حسین فخر، دکتر فریدون مهرابی و روانپزشکان دیگری که توسط دکتر احمد محیط از سراسر کشور دعوت شده بودند.

رسالت یا استراتژی اول این پیش&#8204;نویس، ادغام بهداشت روان در خدمات بهداشتی اولیه[4] بود. ضمناً این برنامه اولین برنامه تخصصی[5] بود که در شبکه بهداشت کشور ادغام شد.

از سویی، در سال&#160;1364، به&#8204;دنبال تصویب قانون تأسیس وزارت بهداشت، درمان و آموزش پزشکی، آموزش رشته پزشکی که تا آن زمان به عهده وزارت علوم بود به وزارت بهداشت، درمان و آموزش پزشکی واگذار شد و در این راستا، دانشگاه&#8204;های علوم پزشکی ایجاد شدند و در سال 1365، انستیتو روانپزشکی تهران، زیرمجموعه دانشکده پزشکی دانشگاه علوم پزشکی ایران شد.

برای اجرایی&#8204;شدن برنامه ملی ادغام بهداشت روان در خدمات بهداشتی اولیه، نیاز به انجام آن به صورت آزمایشی (پایلوت) بود؛ سه شهر انتخاب شدند، یکی شهرکرد به پیشنهاد دکتر بیژن صدری&#8204;زاده (معاون بهداشتی وقت وزارتخانه)، دیگری شهرضا و سومی &#8204;ساوجبلاغ.

هنگامی&#8204;که طرح پایلوت شهرکرد به&#8204;عنوان بخشی از برنامه ملی، با حضور و همت زنده&#8204;یاد دکتر داود شاه&#8204;محمدی در حال انجام بود، دکتر اسماعیل اکبری رئیس وقت دانشگاه علوم پزشکی اصفهان، در سفری به ژنو با دکتر نورمن سارتوریوس[6] که در آن زمان رئیس بخش بهداشت روان سازمان جهانی بهداشت بود، ملاقات کرده و پیشنهاد برگزاری یکی از نشست&#8204;های بهداشت روان سازمان جهانی بهداشت با میزبانی اصفهان را مطرح کرد. به&#8204;دنبال این امر، تصمیم گرفته شد که طرح پایلوت دوم در شهرضا از شهرستان&#8204;های استان اصفهان برگزار شود. دکتر محیط در دو سفر به اصفهان با همکاری جمعی از روانپزشکان دانشگاه علوم پزشکی اصفهان، مقدمات انجام این طرح آزمایشی را فراهم کرد. این مهم، در جریان نشست مذکور مورد توجه چشمگیر شرکت&#8204;کنندگان قرار گرفت. طرح آزمایشی سوم حدود 6 ماه بعد در ساوجبلاغ شروع شد .

در همین موقع، وزارتخانه جدید شروع به برگزاری نشست&#8204;های 15 روزه در استان&#8204;های مختلف کرده بود، هدف از این نشست&#8204;ها تبیین برنامه&#8204;های جدید برای رسالت نوین وزارتخانه بود. در فاصله شش ماهه بین طرح آزمایشی شهرضا و ساوجبلاغ، دکتر محیط برای اجرایی&#8204;کردن برنامه ادغام بهداشت روان در خدمات بهداشتی اولیه در سایر کشور، سفرهایی (در چارچوب نشست&#8204;های 15 روزه) به استان&#8204;های آذربایجان شرقی، آذربایجان غربی، کرمان، فارس، سیستان و بلوچستان و هرمزگان داشت. در نشست شهر کرمان که نحوه اجرای این برنامه ملی توسط دکتر احمد محیط تبیین می&#8204;شد، دکتر جعفر بوالهری به&#8204;عنوان معاون بهداشتی دانشگاه علوم پزشکی اصفهان شرکت داشت. بعدها دکتر جعفر بوالهری در انجام پایلوت ساوجبلاغ نیز نقش به&#8204;سزایی داشتند.

سال 1366 آموزش بهورزها برای انجام برنامه ادغام بهداشت روان در خدمات بهداشتی اولیه آغاز شد و از آنجا که طرح آزمایشی به خوبی در حال پیشرفت بود، به&#8204;طور هم&#8204;زمان در سراسر کشور اجرایی شد.

تا سال 1369، فعالیت&#8204;های یادشده در انستیتو روانپزشکی تهران به ریاست دکتر محیط ادامه یافت تا اینکه او پس از 11 سال از ریاست انستیتو معاف شده و دکتر سید احمد واعظی به این مقام منصوب شد.
