						<title>آیا زمان اجرای آسکی در روانپزشکی رسیده است؟</title>
						<abstract>  آزمون سراسری ارزشیابی پایانی دستیاران روانپزشکی کشور در دو دهه گذشته با وجود کوشش بسیار مراکز آموزش دستیاری روانپزشکی، استادان آزمون‌های سراسری روانپزشکی و دستیاران مربوطه سیری نگران‌کننده داشته و با ناخوشنودی و احساس بی‌عدالتی بسیار همراه بوده است.   در این راستا ضرورت تغییر در شیوه آزمون نظری و بالینی روانپزشکی در چند سال گذشته مورد بحث بوده است. موافقت اعضای هیأت ممتحنه آزمون سراسری روانپزشکی مبنی بر شروع بررسی‌های علمی در زمینه چگونگی تغییر روش آزمون و بررسی آزمایشی آن به روش ”آزمون ساختار یافته عینی - بالینی1“ ( OSCE ) در پایان جلسه آزمون سراسری سال 1384 اعلام شد. هم ‌ چنان ‌ که در متن مقاله‌ها و گزارش ‌ های این شماره دیده می‌شود کوششی همه جانبه و سراسری در راستای جستجو و آزمایش روشی عادلانه، معتبر، پایا، حساس و روز آمد در جامعه آموزش روانپزشکی کشور با همکاری دستیاران روانپزشکی سال ‌ های دوم وسوم روانپزشکی سراسر کشور در سال ‌ های 1384 و 1385 آغاز گردید.   جمع‌بندی یافته‌ها نشان داد که:   1- آزمون بالینی روانپزشکی به روش OSCE ، معتبر، اما پرهزینه و اجرای آن دشوار است.   2- این روش در کشورهای زیادی به‌کار نرفته است اما بهره‌گیری از آن در کشورهایی مانند کانادا، انگلستان، ترکیه و عمان به‌ویژه در مقطع پیش‌فارغ‌التحصیلی گامی نو در ارتقای دانش آموزش روانپزشکی به‌شمار می‌رود.   3- اگر پژوهش و تمرینی بیشتر در سطح ملی و نیز در هر یک از مراکزآموزش روانپزشکی کشور و با حمایت مالی دبیرخانه تخصصی آموزش پزشکی وزارت بهداشت ، درمان و آموزش پزشکی انجام شود، امکان اجرای آزمون سراسری روانپزشکی کشور درسال 1387 به روش OSCE و با ضریب خطای کمترآماده می‌گردد.   4- OSCE به این نیت راه‌اندازی می‌شود که ناخشنودی و احساس بی‌عدالتی را کاهش دهد.   جمع‌بندی نهایی نشان داد که با کوشش دیگری، اما آرام و هوشمندانه می‌توان ایران را با افتخار و سربلندی به گروه کشورهای ارتقاء‌دهنده نظام ارزشیابی بالینی به روش OSCE رسانید.   تصمیم وزارت بهداشت ، درمان و آموزش پزشکی در اجرای اجباری آزمون عملی دانش‌نامه روانپزشکی به روش OSCE درکشور در سال 1386 گرچه نشان داد که دانش و تجربه‌های به‌دست‌آمده در ایران درست و عملی بوده و پیشگامان اجرای OSCE در ایران پایه‌گذاران به حق و ماندگاری در ” OSCE روانپزشکی در ایران و در کشورهای روبه‌‌رشد“ به‌‌شمار خواهند رفت، اما نگرانی ‌ های تازه‌ زیر را برای اجرای آینده این روش پدید آورد:   1- همه طراحان س ؤ ال، اعضای هیأت ممتحنه دانش‌نامه تخصصی، استادان مدرس دستیاری و حتی دستیاران برای اجرای OSCE در روانپزشکی آموزش کافی ندیده‌اند،   2- چون نام استادان هیأت ممتحنه تا دو هفته پیش از آزمون مشخص نیست، گردهمایی وآماده سازی آنان برای طرح سئوال عملی نمی‌باشد، بیماران استاندارد روانپزشکی آماده نیستند، و بودجه ویژه ‌ای برای این کار هزینه‌بر وگران در نظر گرفته نشده است،   3- هنوز روش‌شناسی”نقطه برش“ و نمره پذیرفته‌شده و دقیق ”قبولی و مردودی“ در OSCE روانپزشکی ب ه‌ دست نیامده است،   4- هنوز هستند استادانی که از دانش کم در این روش برخوردارند و با ایجاد دو ایستگاه از نوار مغزی و یک   کلیشه رادیوگرافی در بوق وکرنا می‌دمند که ما سال ‌ هاست OSCE اجرا می‌کنیم و مدرس OSCE در کشور هستیم!!   با این حال این نگران ی که اجرای دوباره آزمون دانش‌نامه تخصصی روانپزشکی به روش OSCE در سال 1387 بر تنش و بی‌اعتمادی بین نظام آموزش روانپزشکی و دستیاران پر تلاش این رشته بیفزاید ، به‌چشم می‌خورد.   مجله روانپزشکی وروانشناسی بالینی ایران با انتشار این ویژه‌نامه و با تبریک به همه کسانی که در ارتقای آموزش روانپزشکی و به‌ویژه همه عزیزانی که با انتشار مقاله‌ها و گزارش ‌ های خود در ارتقای OSCE روانپزشکی کوشیده‌اند، همگان را به آغاز کوشش ی دوباره فرا می‌‌‌خواند تا زمان باقیمانده را به فرصتی نو برای تلاشی دیگر در رفع نگرانی ‌ های گفته‌شده تبدیل نمایند. شاید که دستاوردهای آزمون سراسری دانش‌نامه تخصصی در آینده مورد پذیرش همگان باشد.  </abstract>

						<title>Running the First Psychiatry OSCE in Iran and Satisfaction of Consultants, Residents, and Standardized Patients</title>
						<abstract> Objectives: This study aimed at reviewing how the first Objective Structured Clinical Examination (OSCE) in psychiatry was conducted and at assessing appreciation of this method by the residents, con-sultants, and standardized patients (SPs) compared to the current psychiatry oral exams. Method: A total of 22 psychiatry residents of final year were tested in 9 stations in the presence of 8 standardized patients and a conclusion station. Each station took 12 minutes. Two consultants rated the residents separately in each station based on the designed scoring tables. After the exam, consultants, residents and standardized patients were assessed by questionnaires. This assessment was repeated three weeks later and immediately after the oral board exam from the same residents. Results: Residents described timing of the stations, information given before the exam, and capability of the OSCE in assessing competence of the residents as fair and appropriate. Seventy-five percent of the residents were happy to have participated in this exam. Most of them described the current oral board exam as not fair, not objective and even not similar to the actual clinical settings. Rater consultants also described the goals of the stations, timing, and performance of SPs as appropriate. SPs were happy to participate in this experience again too. Conclusion: Holding a satisfactory and accurate OSCE receives approval of the participants (residents), raters (consultants), and SPs.</abstract>

						<title>ارزیابی کیفی نخستین آزمون ساختاریافته عینی- بالینی (OSCE) روانپزشکی در ایران</title>
						<abstract>چکیده هدف: آزمون ساختاریافته عینی- بالینی (OSCE) روانپزشکی در مرداد ماه سال 1383 نخستین بار در ایران در مقطع تخصصی به‌صورت‌ آزمایشی برگزار گردید. هدف پژوهش حاضر ارزیابی کیفی دیدگاه‌های دستیاران و استادان شرکت‌کننده در این آزمون برای آگاهی از مزیت‌ها و نارسایی‌ها، مشکلات و پیشنهادهای دستیاران و استادان در این زمینه می‎باشد. روش: این بررسی از نوع کیفی و به روش بحث گروهی متمرکز استادان و دستیاران شرکت‌کننده در این آزمون در روز برگزاری آزمون انجام شده است. در این بررسی دو جلسه بحث گروهی با حضور 10- 8  نفر در هر گروه برگزار گردید. یافته‌ها: استادان و دستیاران مهم‌ترین مزیت OSCE را عادلانه‌بودن آن دانسته‌اند. مهم‌ترین نارسایی آن از دیدگاه دستیاران مصنوعی‌بودن فضای ایستگاه و برقرار نشدن ارتباط مناسب با بیمار غیرواقعی بوده است. استادان به هزینه و زمان زیادی که OSCE  نیاز دارد، اشاره نمودند. مهم‌ترین پیشنهاد دستیاران و استادان جهت بهبود OSCE، آموزش بیشتر بیماران استانداردشده و به‌کار بردن فیلم‎های از پیش تهیه‌شده  از بیماران واقعی در برخی از ایستگاه‌ها بود.نتیجه‌گیری: OSCE روانپزشکی از دیدگاه دستیاران و استادان بر شیوه آزمون رایج روانپزشکی دستیاری برتری چشم‌گیر دارد. ولی برگزاری موفق OSCE در سال‌های آینده نیاز به از میان برداشتن مشکلات یادشده از جمله آموزش بیشتر بیماران استانداردشده  و استفاده از سایر پیشنهادهای دستیاران و استادان دارد. </abstract>

						<title>Qualitative Assessment of the First Objective Structured Clinical Examination (OSCE) in Psychiatry in Iran.</title>
						<abstract> Objectives: The psychiatric OSCE was experimentally conducted for the first time in Iran at the level of subspecialties in August 2004. We aimed to qualitatively evaluate the attending professors’ and psy-chiatry residents’ viewpoints in order to understand the advantages, deficiencies, difficulties and suggestions of professors and residents in this regard. Method: In this qualitative study, focused group discussion method of the attending professors and psychiatry residents participating in this test was used on the day of testing. Two focused group discussion sessions were conducted each including 8-10 participants. Results: According to attending professors and psychiatry residents, fairness was the most significant advantage of OSCE. The most im-portant failure of OSCE, according to psychiatry residents, was being artificial in the station environment and lack of an appropriate rapport with the unreal patient. Attending professors pointed out that OSCE is pretty time-consuming and costly. Main suggestions they offered for optimization of OSCE included more further training of standard patients as well as displaying already prepared movies from real pa-tients in some of stations. Conclusion: In professors’ and residents’ point of view, psychiatric OSCE has significant advantages over current testing for psychiatric residents. However, providing a successful OSCE in the upcoming years requires that the aforementioned problems be cleared by further training of standard patients and applying other suggestions cited by the professors and residents.</abstract>

						<title>Validity and Reliability of the First Objective Structured Clinical Examination (OSCE) in Psychiatry in Iran</title>
						<abstract> Objectives: This study aims at the evaluation of validity and reli-ability of Objective Structured Clinical Examination (OSCE) in psy-chiatry and among the residents. Method: Following assessments and a lot of discussions among all consultant psychiatrists in the country, nine 12-minute stations were designed. In each station, two examining raters completed the pre-viously designed checklists individually. Standardized patients were used in 8 of the stations. Face and content validity, construct validity, concurrent validity and inter-rater reliability, internal consistency and inter-station reliability were all assessed. Results: Face and content validity of the test was reached after holding sessions with the presence of the consultants and chair of the country’s specialty board and also providing all practical goals while discussing them in different group meetings. Concurrent validity was evaluated after comparing OSCE scores for both the residents and their own consultants. Concurrent validity was assessed through evalu- ating correlation of OSCE scores with results of oral and written board exams, competency exams, and psychiatry entry exam, which actually showed an appropriate validity for this test. Assessment of test reliability using the inter-rater reliability, internal consistency, and inter-station reliability displayed an appropriate reliability for this OSCE. Conclusion: The first psychiatry OSCE had significant validity and reliability.</abstract>

						<title>Attitude of Psychiatric Professors and Residents towards Performance of Standardized Patients in the Objective Structured Clinical Examination in Psychiatry</title>
						<abstract> Objectives: The increasing use of standardized patients in the psy-chiatric Objective Structured Clinical Examination (OSCE) promoted the investigators to explore the viewpoints of examining professors and participating psychiatric residents towards the performance of the standardized patients. Method: In an OSCE, 22 of the final year psychiatric residents and 16 examiners participated in our study. Performance of the 8 standardized patients in all 8 stations was evaluated after the test. Results: This study showed acceptable and appropriate performance of standardized patients both in acting according to and in actualizing the scenarios. Conclusion: Use of standardized patients in psychiatric OSCE is recommended by the examining professors and also residents. </abstract>

						<title>Are Standardized Patients Able to Rate Performance of Psychiatric Residents in the Objective Structured Clinical Examination?</title>
						<abstract> Objectives: This study aims to compare viewpoints of standardized patients (SPs) and consultant psychiatrists about the performance of volunteer residents in the first Objective Structured Clinical Examination (OSCE) in Iran. Method: OSCE was hold with 9 stations and 8 SPs, in one day with attendance of 22 senior psychiatric residents. In each station two raters independently scored the residents based on a previously prepared checklist. SPs were three females and five males with the mean age of 27 years. In the end, viewpoints of consultant psychi-atrists and SPs on the performance of residents were compared. Results: Beside three stations, no significant correlation between SPs’ and consultants’ opinions could be found in other stations.  SPs in the stations with significant correlation had higher level of educa-tion especially in the psychiatry-relevant fields. Conclusion: Despite capabilities of the SPs, it is probably not reason- able to rely on their opinion as raters unless they have higher level of education.</abstract>

						<title>Attitude of Iranian Psychiatry Residents toward Psychiatric Objective Structured Clinical Examination</title>
						<abstract> Objectives: Few reports are available regarding the use of Objective Structured Clinical Examination (OSCE) for the evaluation of psy-chiatry residents. Assessing their attitudes towards this method of testing has been the aim of this study. Method: Forty psychiatry residents from all universities in Iran who were selected on a stratified sampling method participated in the third comprehensive pilot OSCE in Iran. After the exam, viewpoints were canvassed using a questionnaire and also face to face interview. Data were then analyzed using descriptive statistical methods. Results: Ninety percent of residents believed that the simulations and scenarios were quite realistic and 97.5% considered the scenarios reflective of clinical situations they usually experience. Most of the residents believed also such an examination could discriminate com-petent and incompetent residents successfully. Eighty percent of the residents believed that OSCE is superior than the oral board examina-tion and 87.5% felt they would like the experience to be a formative part of their residency training. Conclusion: The oral or practical board examination can be replaced by OSCE.</abstract>

						<title>Validation of Multidimensional Students’ Life Satisfaction Scale (MSLSS)</title>
						<abstract> Objectives: The purpose of the present study has been assessing life satisfaction as an indicator of positive attitude towards life. Method: Multidimensional Students’ Life Satisfaction Scale (MSLSS) was used for 200 high school students (100 boys and 100 girls) with three educational field of study: mathematics, science, and humanities in districts three and five of Isfahan city. They were selected on a randomized multi-stage cluster sampling. Data were then analyzed using t-test, Pearson correlation coefficient, one-way analysis of variance, and factor analysis. Results: Cronbach’s Alpha in all subjects, male students, and female students were 0.863, 0.844 and 0.884 respectively. Furthermore, the reliability of the instrument was considered acceptable for the educational districts (of three and five) as well as among the three educational study fields (mathematics, science, and humanities). Reliability of five research subscales: friends, self, school, family, and living environment was also reported as acceptable. Factor analysis indicated that items of instrument could be reduced to five factors reflecting construct validity. Conclusion: This scale can be used as a reliable measure in the high school students in Isfahan.</abstract>

						<title>آغاز به‌کارگیری آزمون ساختار یافته عینی- بالینی روانپزشکی در ایران</title>
						<abstract> آغاز کاربرد آزمون ساختار یافته عینی- بالینی1 (OSCE) روانپزشکی در ایران ضرورت بررسی اعتبار و پایایی آن را ضروری می‌نمود. کم‌کردن نقش شانس در انتخاب استاد یا امتحان‌گیرنده یا بیماری که قرار بود مورد مصاحبه قرار گیرد یکی از علت‌های بررسی گسترده‌تر OSCE بود (لیشنر2، سیسلر3 و هارپر4، 1986). افزون بر این OSCE توانایی سنجش دامنه گسترده‌تری از مهارت‌های بالینی و نه یک یا چند مهارت محدود را داشت (اسلوان5، دانلی6 و شوارتز7، 1995). گرچه OSCE در بسیاری از رشته‌های پزشکی به‌عنوان آزمونی استاندارد برای سنجش توانایی‌های بالینی به‌کار برده شده است (هاجز8، هانسون9، مک‌ناتان10 و رگهر11، 2002)، اما در روانپزشکی این روند به آهستگی انجام شد (سایر12، هاجز، سانتوز13 و بلاک‌وود14، 2005).زمان معمول یک مصاحبه روانپزشکی 45-30 دقیقه‌ای امکان فشرده‌شدن در حوزه محدود آزمون OSCE را نداشت و در این میان نگرانی وجود داشت که مهارت‌هایی چون برقراری ارتباط به‌خوبی سنجیده نشود. این دغدغه و برخی پرسش‌های دیگر گسترش OSCE را با محدودیت‌هایی روبرو ساخت (پارک15، چیبنال16، بلاسکی‌ویکز17 و فورمن18، 2004).در این راستا پس از بررسی‌های مقدماتی برای اعتباریابی صوری و محتوایی OSCE روانپزشکی، بیشتر استادان پذیرفتند که نخستین آزمون کشوری آزمایشی ایرانی در مرداد 1383 در تهران برگزار شود. پس از برگزاری آزمون یاد شده، استادان و دستیاران شرکت‌کننده طی یک نظرسنجی، هماهنگی مواد برگ آزمون19 با اهداف ایستگاه، توانایی ایستگاه در اندازه‌گیری مهارت عملی دستیار و نه صرفاً دانش تئوریک او و نقش‌آفرینی بیماران استانداردشده20 (SP) را در بیش از 90 درصد موارد هم‌چون بررسی انجام شده با دستیاران روانپزشکی انگلستان (سایر و همکاران، 2005) خوب گزارش کردند. این موفقیت، بررسی بیشتر این روش را پس از رفع برخی نارسایی‌ها هم‌چون مشکلات اجرایی، آماده‌سازی ایستگاه‌ها، رازداری، مهیاسازی سناریوها و ایستگاه‌ها طی زمانی کوتاه ضروری می‌نمود. دومین کارگاه و آزمون در آذر ماه 1384 در اصفهان برگزار گردید. در این آزمون 22 نفر دستیار شرکت داشتند. تعداد ایستگاه‌ها به‌منظور صرفه‌جویی در زمان از هشت  ایستگاه به شش ایستگاه کاهش داده شد و برخلاف دوره گذشته بیماران استانداردشده از میان استادان انتخاب شدند. در این دوره تهیه سناریوها و برگ‌آزمون‌ها با دقت بیشتری مورد توجه قرار گرفت. گرچه بیماران استاندارد شده در این آزمون از عملکرد خوبی برخوردار بودند اما به‌دلیل شکایت دستیاران از عدم امکان برقراری ارتباط مناسب با استادی که وی را می‌شناختند باعث شد برای دوره‌های آینده، به‌کارگیری استاد به‌عنوان بیمار منتفی گردد.سومین کارگاه، فروردین ماه 1385 در ساری برگزار گردید. شمار ایستگاه‌ها هم‌چون دوره پیش شش ایستگاه تعیین گردید، اما شمار دستیاران شرکت‌کننده به 43 نفر افزایش یافت. آزمون با یک گروه بیماران استاندارد شده و طی یک روز برگزار شد. بیماران از میان دستیاران سال اول روانپزشکی و دانشجویان پزشکی داوطلب انتخاب گردیدند. افزون بر سپاس ویژه از برایان هاجز، یکی از شناخته‌شده‌ترین اندیشمندان این رشته در دنیا که با راهنمایی‌های ارزنده خود کمک زیادی به نویسندگان این ویژه‌نامه نمود، خود را مدیون لطف بسیاری از استادان روانپزشکی کشور، به‌ویژه دکتر غلامرضا میرسپاسی دبیر محترم بورد روانپزشکی کشور می‌دانیم که در زمینه راه‌اندازی و گسترش OSCE کوشش و همکاری بسیاری نمودند. این بزرگواران که بی‌چشم‌داشتی در این راه کوشیدند، شاید بخواهند آن را تلاشی برای فراهم نمودن امکان مساوی آزمون بالینی برای همه دستیاران روانپزشکی بدانند. شاید با این نگرش واگویی جمله‌ای از یکی از دستیاران در ساری بی‌مناسبت نباشد:“هر OCSE بد، بهتر از یک غیرOSCE خوب است“. منابعHodges, B., Hanson, M., McNaughton, N., &#38; Regher, G.
