abstract ={Objectives: This study aims to assess the implementation of drug-related harm reduction programs in Iranian prisons and suggest solutions for their improvement. Methods: This study was conducted in three steps. First, library method was used for collecting data from the central library of Iran’s Prisons, Security and Corrective Measures Organization. In the second step, performance indicators were extracted based on the results of first step and two researcher-made checklists were designed. Finally, a field visit and a semi-structured interview with the authorities involved in the treatment and harm reduction services were carried out. Results: In most of prisons, drug-related harm reduction programs were underway. Despite a lack of human resources and budget at the beginning, the quality of measures was gradually increased and the attitude of authorities was improved. Methadone Maintenance Treatment and Triangular Clinics were the most common harm reduction programs, in addition to HIV and tuberculosis screening programs in collaboration with medical sciences universities. The program continued despite the change of officials. Conclusion: Harm reduction programs are able to reduce infection diseases, self-harm and violent behaviors in prisons of Iran. Cultural programs along with other harm reduction programs, briefings and seeking support from the authorities can greatly help with continuation of the programs in prisons. By eliminating the shortage of manpower and redefining the security areas for ordinary prisoners, it will be possible to make better use of the facilities of universities and research centers.},

title = {From Leaving Home to Losing Identity: A Qualitative Study on Rough Sleeping in Drug Addicts in Iran},
abstract ={Objectives: The phenomenon of rough sleeping in big cities of Iran, particularly in Tehran is a serious social problem. The term rough sleeping is mostly used for those who sleep on the public spaces including pavements, parks or under bridges. It is assumed that rough sleeping and drug addiction are linked to each other. This study aims to evaluate the process of rough sleeping based on the experiences of homeless drug addicted in Iran. Methods: This is a qualitative research. Participants were 20 homeless addicts (9 males and 11 females, mean age= 36.2 years) in Tehran city who were recruited using a purposive sampling method, and sampling continued until data saturation. The data were collected through a semi-structured in-depth interview and observations. Data analysis was performed based on the grounded theory recommended by Strauss and Corbin (1998). Results: The rough sleeping process had two steps: (a) Leaving home voluntarily (to protect family or escape from family problems) or involuntarily (Due to being expelled from home and losing the roof over the head), and (b) Being homelessness which had four main categories: Elusive life, informal and contravened subsistence, involving in exploitative relationships, and loss of individual/social identity. Conclusion: When leaving home and becoming rough sleeper, the individual becomes elusive, delinquent, damaged and socially rejected homeless, in addition to the risk of becoming a drug addict. Therefore, this group can be considered as a severely injured group involved in various social, legal and psychological issues. Given the complexity of the homelessness aspects in drug addicts, there is a need for coordinated interventions between individuals, families and the support systems at all levels.},

title = {Urban Mental Health Initiative: Developing Interdisciplinary Collaboration, the Role of the Family Physician in Management of Severe Mentally Ill Patients},
abstract ={Introduction: Mental health in Iran was approved in 1988. However, and then this program was integrated into the primary care system in our country and was implemented within the villagers' family physician program with great success. In urban areas, there is no structured health network. However, regarding the demographic changes in the country's population, huge sprawling of cities, and the changes in urban-rural population proportion, demands for a coherent plan to provide mental health services to the urban population are felt more than ever. This study aimed to investigate the feasibility and establishment of a model of urban mental health network for severe psychiatric patients. Methods: This plan is based on resource reviews, Use the experiences of other countries, Model World Health Organization, The services available in the country currently run by the beneficiary organizations, Description of the responsibilities of the responsible organizations, Collaboration between the Ministry of Health and various organizations responsible for mental health, Having the views of national expert and international constant in this field from other countries cooperated with the aim of examining how to compile a coherent and integrated urban health service plan. This model is estimated by community-based services for 100,000 people. Results: The proposed model for providing immediate psychiatric services with greater cohesion and increasing training and skills capacity among staff 110, 115, 1480, and 123 services, as well as a space for hospitalization of 3 to 5 emergency patients next to the public hospital, is recommended. Depending on the number of patients in a population of 100,000, we will need community-based services, including 2 to 3 home visit teams to cover 80 patients per team, and 3 daily centers to provide services to 40 patients. If community-based services are provided, we will need 12 acute psychiatric beds and 5 beds for mid-term rehabilitation to provide inpatient services. In terms of employment and accommodation, 50 patients will need supported employment, respectively, and two apartments with an area of about 60 meters will be needed to accommodate about eight people. The provision of the above services requires the equal participation of the interested organizations. The family physician will play an essential role in continuing medical care for severe and mild psychiatric patients under the constant supervision of specialists Discussion and Conclusion: To achieve a better model of mental health services in cities that can cover a wide range of people in urban areas and at various levels from prevention and care to treatment and rehabilitation, we need the coordination between the organizations providing these services for the accurate planning of the interests of each organization, elimination of duplicate services, and saving human capital and resources of the country.},

abstract ={Our subject is a 55-year-old man and a former civil servant. He claimed to have seen someone who others could not see. He believed that he is an important person. He had strange behavior such as separating himself from others, speaking and laughing alone, and collecting rubbish. Based on the assessment through observation, interviews, and psychological tests, the subject was diagnosed with schizophrenia and behavioral problems such as collect things. The subject’s behavior appears because he feels pleasure after collecting goods. He did not do other activities that make him feel good; thus, the behavior was repeated. The intervention aims to reduce the frequency of collecting things through negative reinforcement and rewards techniques. The intervention eventually suppressed the behavior of collecting things carried out by the subject gradually.},

title = {Infidelity a Challenge Facing Therapists},
abstract ={It seems vital to address the problem of extramarital relationships or marital infidelity, which is a traumatic issue for families and spouses and a common issue in counseling and treatment for mental health therapists. This social problem is also very common, however, due to its crucial and personal aspect, accurate information about its prevalence is not available in Iran. Infidelity refers to any concealment of a spouse about an extramarital affair. In fact, secrecy is an important part of an illegitimate and romantic relationship. Types of infidelity include sexual, emotional, and virtual (Internet) infidelity. Men react more strongly to sexual infidelity and women to emotional infidelity. An important point to note is that dissatisfaction with marriage can lead to infidelity, but a successful marriage is no guarantee that it will not happen. Despite its prevalence, not much research has been done in this area. In a US study, integrative intervention therapy was used for couples with such problem and it was found that this integrated treatment model had a good effect on the symptoms of Post-Traumatic Stress Disorder (PTSD). Some tudies have been conducted in Iran despite the sensitivity of the issue. In a systematic review conducted in Mashhad to investigate the causes of infidelity in married women, four causes of individual-personality, cultural-social, family-educational, and religious-belief were more effective than other causes. In another study, Acceptance and Commitment Therapy (ACT) was performed in Tehran to reduce anxiety and depression among women with extramarital affairs and it was observed that this treatment clearly reduces the mentioned symptoms.},

abstract ={Objectives: Despite the studies that examined the effect of exercise and meditation on depression and anxiety, no meta-analysis has proved this effect so far. Also, the effect of these factors is unknown in terms of gender and aerobic exercise. This meta-analysis addressed these gaps. Methods: Major electronic databases, including PubMed, Web of Science, and Embase, were searched until July 2019. Studies addressing the effect of exercise and meditation on depression and anxiety were found. Q-test and I2 statistic investigated the heterogeneity across the studies. The probability of publication bias was explored using Begg's and Egger's tests. The results were expressed as the Standardized Mean Difference (SMD) with a 95% confidence interval using a random-effects model. Results: We identified a total of 10015 references and 51 studies involving 3594 participants. Compared with the control group, the SMD estimate of anxiety was -0.7 (-1.20, -0.19) for meditation, -0.9 (-1.24, -0.57) for aerobic, and -1.07 (-1.67, -0.46) for other exercises. Also, the SMD estimate of depression was-0.84 (-1.26, -0.42) for meditation, -0.44 (-0.72, -0.17) for aerobic, and -1.10 (-1.55, -0.65) for other exercises. Conclusion: Exercise and meditation had a significant effect on depression and anxiety. abstract ={Objectives: Recent studies have identified a disorder called Sluggish Cognitive Tempo (SCT) which has similarities with Attention Deficit/Hyperactivity Disorder (ADHD) in attentional problems and often is mistakenly diagnosed as ADHD. Attentional problems cause executive dysfunction and often affect the hot and cold executive functions. This study aims to compare the sensitivity to reward and punishment, response inhibition and sustained attention between school boys with ADHD and SCT. Methods: This is a casual-comparative study. Participants were 100 elementary school male students (50 with ADHD and 50 with SCT) in Tabriz, Iran in the academic year 2019-20, who were selected through a screening method and based on the Swanson, Nolan, and Pelham-IV (SNAP-IV) and the SCT questionnaires. The Balloon analogue risk task was used to measure the sensitivity to reward and punishment, while the continuous performance test was used to measure response inhibition and sustained attention. For data analysis, multivariate analysis of variance was performed in SPSS v.20 software. Results: The boys with ADHD had higher scores in sensitivity to reward, while those with SCT had higher scores in sensitivity to punishment. Moreover, the boys with ADHD had deficits in response inhibition, while those with SCT had deficits in sustained attention (P<0.0001). Conclusion: It seems that school boys with SCT have deficits in hot executive functions, and those with ADHD have problems in cold executive functions.},

abstract ={Objectives: The Patient Health Questionnaire-9 (PHQ-9) is one of the most commonly used measures for screening, diagnosing and monitoring of depression. So far, four studies have examined some of the psychometric properties of Persian version of PHQ-9 in physical and psychiatric patients; however, some aspects of its psychometrics have not yet been adequately evaluated in the student population. Therefore, this study aims to examine more psychometric properties of the Persian version of PHQ-9 in the student population. Methods: In this descriptive study, 463 students of Guilan University of Medical Sciences in the academic year 2018-19 participated who were selected using a convenience sampling method. The reliability of the Persian PHQ-9 was assessed using internal consistency and test-retest reliability for a 2-week interval. Its construct validity was evaluated by exploratory and confirmatory factor analyses. To evaluate its convergent validity, the second version of the Beck Depression Inventory (BDI-II), the depression subscale of the 21-item Depression Anxiety Depression Inventory (DASS-21), the neuroticism subscale of NEO Five Factor Inventory (NEO-FFI) and the negative affect subscale of the Positive Affect and Negative Affect Scale (PANAS) were used. The openness subscale of NEO-FFI and the positive affect subscale of PANAS were also used to assess its discriminant validity. Results: Cronbach’s alpha coefficient of 0.856 and intraclass correlation coefficient of 0.869 indicated high internal consistency and test-retest reliability of the Persian PHQ-9, respectively. Exploratory factor analysis results showed a one-factor structure, accounting for 47.59% of the total variance. Confirmatory factor analysis results showed that all questions were related to one factor. Positive correlation of the Persian PHQ-9 score with BDI-II (r= 0.769), the neuroticism subscale of NEO-FFI (r= 0.508), the depression subscale of DASS-21 (r= 0.647) and the negative affect subscale of PANAS (r= 0.430) indicated an acceptable convergent validity, while its negative correlation with the positive affect subscale PANAS (r= -0.444) indicated its acceptable discriminant validity; however, its score had no significant relationship with the openness subscale of NEO indicates NEO-FFI (r= 0.116). Conclusion: The Persian PHQ-9 can be used in clinical and research fields to screen, diagnose, and monitor clinical and/or subclinical depression in the Iranian student population. },

This study aims to examine inter-parent agreement and discrepancy on ratings of hyperactivity/impulsivity, inattention, behavior problems, and emotion dysregulation in children with ADHD. Methods: This is a descriptive correlational study. The study population consists of children with ADHD aged 6-10 years who referred to child and adolescent psychiatric clinics in Tehran, Iran. Of these, 51 children were selected using a convenience sampling method. They were separately assessed by their parents using the Swanson, Nolan and Pelham-IV (SNAP-IV) Questionnaire, Eyberg Child Behavior Inventory, Emotion Regulation Checklist, and the Parenting Stress Index- Short Form. Results: Parents had a moderate agreement on all scales/subscales except on emotion dysregulation which was fair. Mothers rated higher than fathers in all areas except for the inattention subscale. Mother’s parenting stress and educational level could predict inter-parent discrepancy on ratings. Conclusion: Mother-father discrepancy on the symptoms and problems of ADHD is clinically significant. abstract ={Objectives: Supporting and managing the emotions of the learning-teaching process is very important and requires designing safe learning environments in terms of cognitive experiences, psychosocial relationships, and emotional feelings. This study aimed to develop and validate of instructional design model based on Control-Value Theory (CVT) of achievement emotions to improve the psychological health and performance of learners and educators. Methods: The research method was qualitative, and we used the inductive content analysis technique. The studies related to CVT theory were searched from 2007 to 2021 in different databases, and 175 articles were selected out of them. Based on the purposive sampling method, 63 articles were chosen for further analysis. In a systematic approach, themes related to essential components of CVT were recognized and coded in the form of key sentences in a table. Then, the codes were categorized based on similarities and differences in general categories. Finally, an instructional design model based on CVT was designed. The model was sent to 20 specialists in the fields of Educational Technology and Educational Psychology content validation. Of them, 12 experts completed the questionnaire, and the results were analyzed. Results: Based on the analyses, the main components of CVT were identified. Then, by combining instructional design principles, the experts presented and approved an instructional design model based on CVT of achievement emotions with eight components: learning environment, cognitive appraisal, emotion, achievement, learning, evaluation, design, and learner. Conclusion: The instructional design model based on CVT of achievement emotions is very effective at the micro and macro levels at all academic levels and various majors such as medicine, humanities, and basic sciences. title = {Developing and Accessing Validity and Reliability of the Persian Versions of Three Instruments Measuring Risky Sexual Behaviors},
abstract ={Objectives: There are a few instruments available to assess sexual issues in Persian according to indigenous culture. This study aims to translate three instruments of Sexual Addiction Screening Test – Revised (SAST-R), Sexual Risk Survey (SRS) and sexual behavior subscale of the Youth Risk Behavior Surveillance System (YRBSS), and evaluate their psychometric parameters. Methods: After translating the selected instruments into Persian, a group of psychiatrists evaluated its content validity. The final versions were distributed among 92 participants (45 men and 47 women) selected from among the companions of patients referred to the clinics of Iran Psychiatric Hospital. Then, the factor structure and internal consistency of them were assessed. Results: The Cronbach’s alpha coefficient for Persian versions of SAST-R, SRS and sexual behavior subscale of YRBSS was obtained 0.878, 0.869 and 0.511, respectively. The factor analysis extracted 12 factors for the SAST-R explaining 71.4% of the variances, 5 factors for the SRS explaining 76.2%b of the variances, and 2 factors for the sexual behavior subscale explaining 56.7% of the variances. Conclusion: Persian version of the SAST-R has high internal consistency with a complex factor structure. The Persian version of SRS has high internal consistency with a well-defined factor structure, while the sexual behavior subscale of YRBSS has a good factor structure with a moderate internal consistency. These instruments can be used on Iranian population. Further studies are recommended to evaluate their construct validity and reliability.},

abstract ={Objectives: This study aims to evaluate the efficacy and safety of adding pregabalin to standard antipsychotic treatment in patients with chronic schizophrenia. Methods: In this randomized, double-blind, placebo-controlled clinical trial, participants were 48 male inpatients aged 18-65 years with chronic schizophrenia but with clinically stable conditions. They were randomly divided into two groups of intervention (n=24) and control (n=24). The intervention group received 75 mg of pregabalin per day for three weeks which was increased to 150 mg per day from the fourth week to the end of the study, in addition to standard antipsychotic medication for six weeks. The severity of psychotic symptoms was assessed by the Positive and Negative Syndrome Scale (PANSS), and their movement disorder symptoms were evaluated by the Barnes Akathisia Rating Scale (BARS) and Simpson-Angus Scale (SAS) at baseline and at 3th and 6th weeks of the intervention. Collected data were analyzed using repeated measures ANOVA in SPSS v.20 software, considering P<0.05 as the statistically significance level. Results: No significant differences were observed in severity of disease based on the PANSS total score and its three subscales (positive scale, negative scale, and general psychopathology) and in the BARS and SAS scores at the end of 6th week in groups and between the two groups. Conclusion: Adding 150 mg of pregabalin to the standard antipsychotic treatment of patients with chronic schizophrenia is not effective in improving their psychotic symptoms, but it is well tolerated.},

abstract ={Objectives: This study aims to compare the effects of two methods of motivational manipulation and neurofeedback on reward sensitivity, delay discounting, and impulsivity in children with Attention Deficit/Hyperactivity Disorder (ADHD). Methods: This is an experimental study with a pre-test/post-test/follow-up design using a control group. Participants were 90 children aged 7-12 years with ADHD in Tehran, Iran who were selected using a convenience sampling method. Neurofeedback and motivational manipulation were performed in the two intervention groups at 12 sessions each session for 45 minutes. Balloon Analogue Risk task and Chocolate Delay Discounting task were used to collect data. The collected data were analyzed using MANCOVA in SPSS v.23 software. Results: After controlling the effects of pre-test scores on post-test scores, the difference between the groups in the post-test phase was statistically significant in terms of impulsivity and delay discounting (P<0.01) and reward sensitivity (P<0.05). The neurofeedback method (Mean=4.66) had a greater effect on reward sensitivity than motivational manipulation method (Mean=2.31) compared to the control group, which was significant (P<0.01). The effect of motivational manipulation (Mean=2.31) was not significant in comparison with the control group. Conclusion: Voluntary activation of dopaminergic regions of the brain by neurofeedback and motivational manipulation leads to endogenous dopamine control in these regions, resulting in successful regulation or inhibitory control and reduced cravings, which reduces impulsivity and reward sensitivity.},

title = {Anxiety Symptoms Associated With the Severity of Problematic Smartphone Use: The Mediating Role of COVID-19 Anxiety},
abstract ={Objectives: The Coronavirus Disease 2019 (COVID-19) pandemic is regarded as the biggest global health crisis in recent decades. The changes in major life domains due to infection control strategies resemble the functional impairment consequential to emotional distress and place many people at greater risk of psychiatric conditions. Meanwhile, the COVID-19 pandemic and associated social distancing have widely increased the usage of smartphone to receive COVID-19-related information in the past year. However, the excessive use of smartphones has deleterious consequences. The current study investigated general anxiety symptoms and COVID-19-induced anxiety, and their impact on problematic smartphone use (PSPU) severity. Methods: Adults aged 18–75 years were recruited by stratified, probability sampling from a database representative of the population. Among 1080 Hong Kong Chinese (age 18–75 years), a cross-sectional web-based survey was conducted between July 23 and August 25, 2020 (the period of the third wave outbreak of COVID-19 in Hong Kong). Participants were assessed with the measures, including the Depression Anxiety Stress Scale-21, Generalized Anxiety Disorder Scale-7, and Smartphone Addiction Scale-Short Version. Structural equation modeling (SEM) was used to evaluate the associations among general anxiety symptoms, COVID-19-induced anxiety, and severity of PSPU by the Amos 22.0 software. Results: Using established cut-off scores, our results found that prevalence of at least moderate anxiety level in around 22% of participants. Using SEM, both general anxiety symptoms and COVID-19-induced anxiety were associated with PSPU severity. Moreover, COVID-19 anxiety partially mediated relations between predisposing general anxiety with PSPU severity. Conclusion: This study provides initial data on the mental health status of people affected by COVID-19. It also investigated the relations between COVID-19-induced anxiety and PSPU severity as a coping mechanism. },
