مقایسه کارکردهای اجرایی مغز در کودکان دارای اختلال بیشفعالی و نقص توجه با کودکان دارای اختلال بیشفعالی نقص توجه همراه با اضطراب
Comparing Executive Cognitive Functions of Brain in Two Groups of ADHD Children With and Without Anxiety

<p dir="RTL" style="text-align: justify;"><strong>اهداف</strong> هدف پژوهش حاضر مقایسه کارکردهای اجرایی مغز در دوگروه از کودکان دارای اختلال نقص توجه/ بیش‎فعالی دارای اضطراب یا فاقد اضطراب بود. <br>
<strong>مواد و روش ها</strong> در این مطالعه مقطعی-مقایسه‎ای، 15 کودک و نوجوان مبتلا به بیش‎فعالی/ نقص توجه بدون اضطراب و 15 کودک و نوجوان مبتلای دارای اضطراب، از بین بیماران مراجعه‎کننده به مراکز روان‎پزشکی سه منطقه تهران (مناطق یک، سه و نُه) و با توجه به نتایج حاصل از تکمیل مصاحبه نیمه ساختاریافته تشخیص اختلالات روانپزشکی کودک ونوجوان (K-SADS)، پرسش‎نامه کانرز-فرم والد و مقیاس تجدیدنظرشده اضطراب آشکار کودکان (RCMAS) انتخاب شدند. سپس آزمون‎های برج لندن و ویسکانسین در دو گروه اجرا شدند و نتایج حاصل با استفاده از آزمون تحلیل واریانس تک متغیری مورد ارزیابی قرار گرفت.<br>
<strong>یافته ها</strong> یافته‎ها نشان دادند که میان کارکردهای اجرایی مغز کودکان (قدرت حل مسأله و تصمیم‎گیری؛ انعطاف‎پذیری ذهنی و تفکر انتزاعی؛ قدرت مهار و تکانش‎گری) دارای اختلال نقص توجه/ بیش‎فعالی و دارای نقص توجه/ بیش‎فعالی همراه با اضطراب، تفاوت وجود دارد (05/0=p).</p>
<p dir="RTL" style="text-align: justify;"><strong>نتیجه گیری</strong> اضطراب بیمارگون موجب کاهش خطرپذیری، عدم مپهار و تکانش‎گری در افراد دارای اختلال نقص توجه/ بیش‎فعالی می‎شود و تفکر انتزاعی را بهبود می‎بخشد. اما در قدرت تصمیم‎گیری و حل مسأله این افراد، اختلال جدی ایجاد می‎کند.</p>
<p style="text-align: justify;"><strong>Objectives</strong> This study was conducted to compare the executive cognitive functions of the brain in two groups of Attention Deficit Hyperactivity Disorder (ADHD) children with and without anxiety.<br>
<strong>Methods</strong> In this cross-sectional comparative study,15 children with ADHD and 15 children with ADHD and anxiety were compared in terms of executive functions. The study population was chosen from patients in three region of Tehran city (one, three and nine regions) through clinical interviews and questionnaires (Kiddie-Schedule for Affective Disorders and Schizophrenia (K-SADS), ADHD Rating Scale, Kaners, Revised Children's Manifest Anxiety Scale (RCMAS). The executive functions were evaluated using the Tower of London Test (TOL) and Wisconsin Card Sorting Test (WSCT). <br>
<strong>Results</strong> The results showed a significant difference between ADHD children with and without comorbid anxiety (P=0.05) in terms of executive cognitive functions of the brain (problem-solving and decision making, impulsivity, and abstract thinking). <br>
<strong>Conclusion</strong> In this study, it was found that children with ADHD and anxiety are better in abstract thinking and flexibility and impulse control (they had fewer errors in WSCT) because anxiety can control risk-taking behaviors and impulsivity, but they have more deficits in decision making.</p>
