fa ارزشیابی برنامه ادغام بهداشت روان در سامانه مراقبت‌های بهداشتی اولیه ایران Evaluation of Mental Health Program Integration into the Primary Health Care System of Iran روانپزشکی و روانشناسی Psychiatry and Psychology پژوهشي اصيل Original Research هدف: هدف پژوهش حاضر، ارزشیابی برنامه ادغام بهداشت روان در سامانه مراقبتهای بهداشتی اولیه (PHC) بود. روش: پژوهش توصیفی- مقطعی حاضر به دو روش کمی و کیفی انجام شد. 1209 نفر از جمعیت عمومی، 146 بهورز، 35 کاردان و 51 پزشک عمومی مراکز بهداشتی- درمانی مناطق زیر پوشش و غیرزیر پوشش بهداشت روان دانشگاههای علومپزشکی گیلان، تهران، مشهد، ایران، کرمان و کرمانشاه، بهصورت چندمرحلهای و به روش تصادفی مورد بررسی قرار گرفتند. شش پرسشنامه آگاهی و نگرش بهورزان در مورد بیماری‌های روانی؛ عملکرد بهورزان در مورد آموزش، بیماریابی، پی‌گیری و ارجاع بیماران مبتنی بر برنامه ادغام بهداشت روان؛ آگاهی و نگرش جمعیت عمومی نسبت به بیماری‌های روانی، صرع و عقب‌ماندگی ذهنی و علل بروز این بیماری‌ها و روش‌های درمانی آنها؛ آگاهی پزشکان عمومی نسبت به درمان، ارجاع و ادغام برنامه بهداشت روان در PHC؛ عملکرد پزشکان عمومی مراکز بهداشتیدرمانی و بررسی کیفی در گروه متمرکز برای گردآوری دادهها بهکار رفتند. دادهها با روشهای آمار توصیفی و آزمون t مستقل تحلیل شدند. یافته‌ها: تفاوت دو گروه بهورزان زیر پوشش و غیر زیر پوشش در آگاهی (01/0≥p) و نگرش (04/0≥p)، نیز تفاوت دو گروه جمعیت عمومی زیر پوشش و غیر زیر پوشش در آگاهی (001/0≥p) و نگرش (001/0≥p) معنادار و میزان بیماریابی بهورزان در خانواده‌های زیر پوشش 25/8 در 1000 نفر جمعیت بود. درحالیکه آگاهی بهداشت روان پزشکان عمومی در مناطق زیر پوشش بالا بود، آنان در 3/34 و 6/48 درصد موارد بهترتیب در تشخیص و درمان بیماران روانپریش مشکل داشتند. نتیجه‌گیری: ادغام برنامه بهداشت روان در PHC، روشی مؤثر و بهصرفه است، که با نظارت و ارزشیابی مستمر، میتواند به پیامدهای ارزشمندی بینجامد. <a name="OLE_LINK4"></a><a name="OLE_LINK3">Objectives</a>: The aim of the current study was to evaluate the integration of mental health program into primary health care system. Method: In a cross-sectional, quantitative and qualitative study, the knowledge, attitude and performance of 1209 individuals of general population, 146 behvarzes, 35 health technicians and 51 general practitioners from rural health centers of Gilan, Tehran, Meshad, Iran, Kerman and Kermanshah universities of medical sciences who were undercoverage and out of coverage of mental health program in the rural areas of selected centers, were selected by randomized cluster sampling. The data was gathered using knowledge, attitude and performance questionnaires of behvarzes, general population and general practitioners toward mental health, behvarzes’ performance on training, following up and referral of mentally patients based on primary health care (PHC) program, knowledge and attitude of general population toward mental illnesses, epilepsy, mental retardation, their  causes and treatments, general physicians awareness about treatment, referral and integration of mental health program into PHC, general practitioners’s performance of health centers and a semi-structured questionnaire for focus group discussion sessions (FGD). Thirty trained mental health experts from neighborhood of selected provinces, completed questionnaires and analyzed. Descriptive statistics and independent t-test were executed. Results: There was significant difference between behvarzes in areas under coverage and out of coverage in knowledge (p≤0.01) and attitude (p≤0.04), as well as between general population in areas under coverage and out of coverage in knowledge (p≤0.001) and attitude (p≤0.001). Although the knowledge of general practitionners in the coverage areas were high, the findings showed that 34.3% of them had difficulty in diagnosis and 48.6% had problem in treatment of psychotic patients.
