fa مشکلات روانپزشکی در درمانگاه‌‌های عمومی: شیوع و علت مراجعه Psychiatric Problems in General Outpatient Clinics: Prevalence and Reasons for Help Seeking روانپزشکی و روانشناسی Psychiatry and Psychology علمي كوتاه Short Scientific هدف: هدف این پژوهش، بررسی شیوع مشکلات روانپزشکی و علت مراجعه مبتلایان در مراکز بهداشتی‌‌درمانی منطقه 17 تهران می‌‌باشد. هم‌چنین این مسأله بررسی شد که وجود شکایات مشخص روانپزشکی یا گزارش سابقه بیماری روانپزشکی توسط خود فرد تا چه حد میتواند پیشبینیکننده وجود مشکل روانپزشکی در مراجعان به این مراکز باشد. روش: این مطالعه یک بررسی مقطعی و توصیفی بود. دو مرکز بهداشتی‌‌درمانی در منطقه 17 تهران به‌‌طور تصادفی از بین مراکز این منطقه انتخاب شدند. مراجعهکنندگان به درمانگاه این مراکز، به صورت پیاپی وارد شده و با پرسش‌نامه سلامت عمومی (GHQ-28) و پرسش‌نامه علت و مسیر مراجعه ارزیابی شدند. یافتهها: 126 نفر وارد مطالعه شدند (54% مؤنث). بر اساس GHQ، 42 نفر (3/33%) دچار مشکل روانپزشکی بودند. علت مراجعه تنها 11 نفر (7/8%)، شکایت مشخص روانپزشکی بود و 60 نفر (6/47%) خود را بیمار روانپزشکی میدانستند. 27 نفر (2/64%) از افرادی که بر اساس GHQ دچار نوعی مشکل روانپزشکی بودند، چه در گذشته و چه در نوبت اخیر مراجعهای برای مشکل روانپزشکی نداشتند. حساسیت وجود شکایات روانپزشکی و گزارش سابقه بیماری روانپزشکی توسط مراجعان، برای تشخیص مشکل روانپزشکی کمتر از 70% و قدرت پیش‌بینی مثبت آنها کمتر از 55% بود. نتیجه‌‌گیری: مشکلات روانپزشکی در درمانگاههای عمومی شایع است، با این حال، اکثر مبتلایان هیچ مراجعه روانپزشکی ندارند. به‌علاوه، اتکای صرف به شکایت اصلی و اظهار خود فرد مبنی بر وجود مشکل روانپزشکی، روش مطمئنی برای غربال اختلالات روانپزشکی در مراکز مراقبت‌های اولیه نیست. <a name="OLE_LINK4"></a><a name="OLE_LINK3">Objectives</a>: The aims of the present study are to examine the prevalence of psychiatric problems and the reasons for seeking care in urban health centers in district 17 of Tehran (capital of Iran), and to study the sensitivity and predictive value of the presence of psychiatric symptoms as chief complaint, and self-report of a psychiatric history for correct recognition of a psychiatric problem. Method: The present research was a cross-sectional and descriptive study. Two urban health centers located in district 17 in <place w:st="on"><city w:st="on">Tehran</city></place> were randomly selected from a total of 7 health centers. Patients referring to outpatient clinics of the centers were consecutively asked to fill in General Health Questionnaire-28 and a pathways-to-care questionnaire. Results: In total, 126 subjects (54% female) were included. Forty-two patients (33.3%) had psychiatric problem based on GHQ. Only 11 (8.7%) had psychiatric symptoms as chief complaints (reasons for referral) and 60 (47.6%) considered themselves having a psychiatric illnesses. Of all GHQ cases, 27 (64.2%) had never consulted for a psychiatric illness. The sensitivity of self-report of psychiatric complaint or illness was less than 70% with positive predictive values of less than 55%. Conclusion: In line with previous research, psychiatric disorders were commonly observed in outpatient clinics in <city w:st="on"><place w:st="on">Tehran</place></city>. However, most of those with a problem had not sought care.
