Introduction
Chronic kidney disease (CKD) is a progressive and irreversible disorder. In this disease, the kidney’s ability to dispose of metabolic wastes and maintain fluid and electrolytes is impaired, leading to the increase of urea and other toxic substances in the blood [12]. The global prevalence of CKD is 13.4% (11.7-15%), and 4.902-7.083 million people need renal replacement therapy or dialysis [5]. The CKD is more common in women than in men [6]. One of the psychological problems in dialysis patients is body image dissatisfaction [8]. Chronic diseases such as heart problems, diabetes and kidney problems cause changes in the shape and appearance of the body, often resulting in body image dissatisfaction [9]. Body image is defined as a person’s perceptions and attitudes towards her/his body, and is a multidimensional concept that includes behavioral, perceptual, cognitive and emotional aspects [10]. Body image satisfaction is directly related to self-compassion [12]. Studies indicate a low level of self-compassion among patients with CKD [14]. According to Neff, the lack of self-compassion can lead to self-criticism, isolation and over-identification with thoughts and emotions. The ability to have self-compassion, on the other hand, shows self-kindness in difficult situations, recognizing one’s shared humanity, and being mindful when exposed to negative thoughts and emotions [16].
Patients undergoing dialysis suffer from many physical and psychological problems and the type of dialysis has no effect on their problems [4, 18]. Identifying effective psychological interventions to improve body image and self-compassion in dialysis patients is necessary for providing health care services to them. Acceptance and commitment therapy (ACT) tend to increase psychological flexibility and emphasizes six components of acceptance, cognitive diffusion, self as context, contact with the present moment, values, and committed action. This approach focuses on accepting difficult inner and outer experiences, adopting a more compassionate and kind way towards oneself, and having action towards living a valuable life. Evidence shows that ACT is effective in managing the problems of patients with chronic pain [20]. Considering that kidney problems are more common among women than men and dialysis women suffer from body image dissatisfaction and lack of self-compassion more than healthy women, this study aims to determine the effect of group ACT on body image and self-compassion of women undergoing dialysis.

Methods
This is a quasi-experimental study (randomized clinical trial) with a pre-test/post-test/follow-up design. Participants were 30 dialysis women referred to Dialysis Patients Support Association in Khorasan Razavi, Iran, who were selected using a convenience sampling method and randomly assigned to two groups of intervention and control. The research tools included body image concern inventory (BICI) and self-compassion scale (SCS). The intervention group received ten 120-minute sessions of group ACT weekly, but the control group did not receive any intervention until the end of the two-month follow-up phase. Data were analyzed using the repeated measures analysis of variance (ANOVA) in SPSS software, version 25. 

Results
The mean age of participants was 48.32±5.32 years in the intervention group and 48.93±4.34 years in the control group. The mean dialysis duration was 7.2±3.8 months in the intervention group and 5±2.75 months in the control group. The results of the multivariate ANOVA showed that the within-group effects (Wilks’ Lambda=0.342, F=12.019, P=0.001), between-group effects (Wilks’ Lambda=0.71, F=5.508, P=0.01), and interaction effects (Wilks’ Lambda=0.29, F=15.267, P=0.001) were significantly different.
Based on the results in Table 1, there were significant differences in within-group, between-group, and interaction effects on body image (P<0.05).


Regarding self-compassion, the within-group effect were not significant (P>0.05), but the between-group and interaction effects were significant (P<0.05). The effect size of the intervention’s interaction effects on body image and self-compassion were 0.584 and 0.208, respectively, which means that 58% of the changes in body image and 21% of the changes in self-compassion during the test were caused by the intervention. The results of Bonferroni post hoc test demonstrated that the mean difference in both variables of body image and self-compassion was significant in the intervention group from the pre-test to the post-test phase and from the pre-test to the follow-up phase (P<0.05), but no significant difference was found from the post-test to the follow-up phase (P>0.05).

Conclusion
The results indicated that ACT had a significant effect on increasing self-compassion and reducing body image dissatisfaction in women undergoing dialysis. Due to the application of techniques derived from ACT such as mindfulness and cognitive diffusion, the women were able to communicate with their experiences in the present moment and be kind and compassionate towards themselves, instead of ruminating and blaming themselves for their difficult situation in the present moment and accept their unpleasant feelings, thoughts and bodily sensations without judgment and not to define themselves according to their sufferings. According to the results, in addition to conventional medical treatments, it is recommended to use ACT to reduce body image dissatisfaction and enhance self-compassion in women undergoing dialysis
